Background:
Mixing valves are used to provide hot and cold mixed water to select exam room lavatories. The distance from the mixing valve to the lav must be short enough to allow only a short period of time to elapse before mixed water is delivered.

Lessons Learned:
The mixing valves need to be located close enought to satisfy providers with correct temperature water. Change order resulted in adding more mixing valves to satisfy providers.

Recommended Action:
During design, review locations of mixing valve to assure there are adequate numbers of valves and the distance to lavatories is within design parameters.
