The Community Health Representatives at the Rapid City Service Unit campus is a 638 program funded by the Oglala Sioux Tribe.

  Between 8:00AM and 4:00PM Monday through Friday, Community Health Representatives provide transportation to and from medical appointments for disadvantaged American Indian population living in Rapid City, South Dakota.  To schedule transportation, call the Community Health Representatives office at least twenty-four (24) hours prior to the appointment time.
