Patient Registration

  Patient Registration is part of the Business Office and updates all established patients and does new patient registration.  They assure maximal identification of persons who have or are eligible for alternate resources by doing face-to-face interviewing and refering patients to the Patient Benefit Coordinator.  Patient Registration will obtain copies and signatures on all documentation needed to bill third party resources and establish a patient's medical chart.  They are responsible to key-enter all required information in the hospital computer database.

Forms of Identification needed to establish eligibility.

  1. One of the following: Birth Certificate, Valid Drivers License, Military Identification, or Passport.
  2. Social Security card.
  3. Tribal Enrollment card (or proof of descendancy from an enrolled tribal member).
