Eligibility

To be eligible for care at W. W. Hastings Hospital, and to establish a new chart, an individual must provide the following documents:

Adult:

  • CDIB/Tribal enrollment card/Tribal roll verification (degree or quantum must be noted on the document)
  • Social Security Card
  • Driver’s License or Picture I.D.
  • Third Party resource card/information (if applicable)

Dependent Children:

  • Child’s CDIB/Tribal enrollment card/Tribal roll verification or parent(s) CDIB/Tribal enrollment card/Tribal roll verification
  • State certified birth certificate
  • Child’s Social Security card
  • Immunization record (if applicable)

Non-Indian Dependent:

  • CDIB/Tribal enrollment card/Tribal roll verification of eligible Indian guardian/foster parent/adoptive dependent children legal documents proving guardianship/adoption
  • State certified birth certificate
  • Child’s Social Security card
  • Immunization record (if applicable)
It is important to make every effort to obtain the child’s own CDIB.

Non-Indian Prenatal:

  • Baby’s Father’s CDIB, Social Security card, and Driver’s License
  • Patient’s Social Security card and Driver’s License
  • Divorce decree (if previously married)
  • Marriage License or acknowledgement of Paternity (both individuals MUST be present). If the male (father) is seventeen (17) or younger, his parent/guardian MUST be present to give permission for acknowledgement.
  • Proof of positive pregnancy test from outside provider

If an individual has used an IHS facility previously, Eligibility may be establish by calling that IHS clinic/hospital the patient has used, or by calling the patient’s tribe to verify enrollment. It is the patient’s responsibility to produce his/her CDIB.
