This transmittal forwards Part 4, Chapter 3, Section 4 of the Indian Health Manual. This section establishes the manner in which the Patient Identification Card is to be prepared throughout the Indian Health Service.

BACKGROUND:
This transmittal forwards Part 4, Chapter 3, Section 2, of the Indian Health Manual. This section establishes Indian Health Service policy, procedures, scope and responsibilities of reporting direct inpatient medical services provided to Indian and Alaska Native people and other authorized patients. These new procedures obsolete the use of Forms PHS-2515-1 (HSM-44-1) "Clinical Record Brief" (manifold) and PHS-2515-2 (HSM-44-2) "Inpatient Record" by substituting a new "Clinical Record Brief - IHS Inpatient Services", Form HSM-44-1 (Rev. 7-71).
A draft Indian Health Manual issuance of Part 4, Chapter 3, Section 1, dated April 27, 1970 (TN No. 70) was used in conjunction with the new "Clinical Record Brief - IHS Inpatient Services", Form HSM-44-1 (Rev. 5-70). The attached revised Indian Health Manual issuance and the revised "Clinical Record Brief - IHS Inpatient Services", HSM-44-1 (Rev. 7-71) are effective immediately.
The direct inpatient medical service section of the Indian Health Manual has been changed from Part 4, Chapter 3, Section 1, to Part 4, Chapter 3, Section 2.
An initial supply of Forms HSM-44-1 (Rev. 7-71) will be forwarded from the usual production sources. Future requests should be submitted in the customary manner.
