Use this checklist to learn more about and get involved in your SDPI Community-Directed grant.
- Determine your budget cycle: Knowing your program’s budget cycle or period will help you and others know what reports are forthcoming. Your program’s budget period will be provided on the first page upper left hand corner of your NOA. Your program will be one of the following four budget periods or cycles:
Find your grant number: Referencing your program’s grant number will help you and others find your program more efficiently. This number can also be found on the first page upper left hand corner of your NOA. Due to recent changes in grant numbers, check GrantSolutions
to confirm your program’s latest grant number.
Read through the Terms and Conditions: The Terms and Conditions provide information and requirements about the business and programmatic aspect of the grant.
- Cycle 1: October 1 – September 30
- Cycle 2: January 1 – December 31
- Cycle 3: April 1 – March 31
- Cycle 4: June 1 – May 31
- Division of Diabetes: Manages the programmatic aspect of your grant including the content of your reports, Best Practices, project narrative of your applications, and this website. Contact us at IHSDDTPSDPICommunity@ihs.gov.
- Area Diabetes Consultant (ADC): ADCs are located in your IHS Area and can provide programmatic and clinical assistance with your grant. Your ADC works more closely with your program so you will want to contact them regarding questions that are more specific to your program. Find your ADC’s contact information on the ADC Directory.
- Division of Grants Management (DGM): Manages the business aspect of your grant including content of the standard forms (SF), grants policy, and GrantSolutions. You can contact DGM at firstname.lastname@example.org.
Your latest Notice of Award (NOA) Terms and Conditions will also provide contact information for your program’s Grants Management Specialist (GMS) and ADC.
- Training: SDPI grantees are required to attend and track trainings offered by the Division of Diabetes specific to their grant program as stated in the Terms and Conditions. You can find more information on trainings specific to your grant program by frequently visiting the SDPI Required Training webpage. For questions, emai IHSDDTPSDPICommunity@ihs.gov.
- Due Date Table: Download the SDPI Community-Directed Timetable [PDF - 103KB] and check with your staff, ADC and your GMS along with your program’s cycle on the SDPI Hub to be sure your program is current with all required reports/documents.
- Best Practices: Review your current application and get familiar with your program’s selected Best Practice(s) for this budget period.
