How To Apply

I.H.S. Pharmacy Residency 2012 Application Checklist

Application deadline: Your resume, transcripts, hiring preference and suitability forms must be uploaded on USA JOBS by midnight Eastern time 1/13/2012. All supporting materials must be emailed or time stamped/mail postmarked by midnight 1/13/2012

EACH ITEM ON THIS CHECKLIST IS REQUIRED

Incomplete or late applications will not be considered. It is the applicant’s responsibility to follow-up with the Residency Director and I.H.S. Recruiter to ensure all application materials are received.

All applications will be considered for the Civil Service (or tribal) employment systems UNLESS the applicant ALSO applies separately to the USPHS Commissioned Corps.

To apply for the Commissioned Corps:
Complete the PHS-50 application for the USPHS Commissioned Corps, beginning with the prescreening questionnaire Exit Disclaimer – You Are Leaving www.ihs.gov that will help determine your eligibility.

If you have a successful prescreen, continue on to the Commissioned Corps application, Exit Disclaimer – You Are Leaving www.ihs.gov following the directions closely (again, this process is optional - it is NOT required to apply for residency).

ALL APPLICANTS:

A. Submit the following documents (#1-5 below) via EMAIL directly to the Residency Program Director (RPD) at each facility to which you are applying. Email addresses can be found via the program links.

___1. Letter of Intent, addressing why you wish to complete an IHS Residency
___2. Technical (medical/scientific) writing sample (not a presentation) from pharmacy school career
___3. Completed IHS Residency Site Selection form (XLS - 30K)
___4. Completed Prospective Resident Survey (DOC - 35K)
___5. Most current Curriculum Vitae
___6. Email completed “Residency Site Selection 2012” form (same as #3 above) to Rebecca.garner@ihs.gov
___7. Fill out on-line USA JOBS application Exit Disclaimer – You Are Leaving www.ihs.gov between November 1, 2011 and Jan 12, 2012**

  • If you wish to be considered as USPHS Commissioned Corps Officer candidate, go to this link Exit Disclaimer – You Are Leaving www.ihs.gov to apply on-line
  • If you wish to be considered under both employment systems, you MUST apply to both announcements on USA JOBS.

ALL APPLICANTS:

B. All materials must be postmarked by midnight January 13, 2012.

___8. Four reference forms (for references listed in section 21 of the PHS-50). Provide each reference an addressed and stamped envelope to mail the completed forms to the address below or include the sealed envelope (signed across the envelope seal) with your application packet. They must be on the PHS-1813 form. We recommend that you collect sealed reference forms that have original signature across outside of seal and send all four references together as one packet to LCDR Killsback, rather than having the writer mail them individually. Emailed references will NOT be accepted.
