Report Fraud, Waste, Abuse and Mismanagement
All employees, contractors and anyone who has contact with the Indian Health Service (IHS) can combat fraud, waste, abuse, and mismanagement. We encourage you to report these matters to the IHS Program Integrity and Ethics Staff OR the Department of Health Human Services Office of Inspector General (OIG) .
Health Information Privacy: If you believe that a covered entity has violated your (or someone else's) health information privacy rights or committed another violation of the Privacy or Security Rule, you may file a complaint with the Service Unit CEO or designee or report it to the HHS Office of Civil Rights (OCR). OCR can investigate complaints against covered entities. For more infomation, please visit the HHS Health Information Privacy web page .

This system contains information protected under the provisions of the Privacy Act of 1974 (Public Law 93-579). Any privacy information displayed on the screen or printed must be protected from unauthorized disclosure. Employees who violate privacy safeguards may be subject to disciplinary actions and/or a fine of up to $5,000, or both.
The purpose of the collection of this information is defined under the Privacy Act of 1974 and the New OIG Privacy Act System of Records: Administrative Files (09-09-0076).
