Search, Sort and Download tips  click on icon to open help Search Code (autotextsuggest field), Provider (empty field), Status (drop down list), Wl Flg (drop down list), Prm Prov Flg (drop down list) and Definition (empty field) columns only or any combination of these columns.

Autotextsuggest pulls values from the field in the database that begin with the string (not case sensitive) entered by the user and displays them as drop down list options. These values populate as the user types. Drop down list searches for results that are an exact match with the value selected by the user. From drop down list, select value 'Empty' to select results where the field is not populated. Empty field searches for results that begin with, include or end with the string (not case sensitive) entered by the user.

D6  APRN PSYCHIATRIC NURSING SPECIALTY  Active  An individual who is licensed/credentialed by a state as an independent practitioner (nurse practitioner) may diagnose disease and provide appropriate treatment including prescribing medications up to and including Schedule II Narcotics. Must have a DEA certification and number to write prescriptions for controlled substances. 
28  AUDIOLOGIST  Active  An individual who has a master’s degree from an audiology program approved by a nationally recognized professional accrediting body; who has completed a supervised clinical fellowship year and passed a national examination in audiology or has the documented equivalent in education, training, or experience; and who meets any current legal requirements for licensure. A specialist in evaluation, habitation, and rehabilitation of those whose communication disorders center in whole or in part in the hearing function. 
89  AUDIOLOGY HEALTH TECHNICIAN  Active  A person trained in and expert in the performance of audiology technical procedures who assists the ENT practitioner. 
43  AUDIOMETRIC TECHNICIAN  Active  Audiometric tests can be performed by a technician who is certified by the Council of Accreditation in Occupational Hearing Conversation, or who has satisfactorily demonstrated competence in administering audiometric examinations, obtaining valid audiograms, and properly using, maintaining and checking calibration and proper function of the audiometers being used. A technician who operates microprocessor audiometers does not need to be certified. A technician who performs audiometric tests must be responsible to an audiologist, otolaryngologist or physician. 
D2  BEHAVIOR ANALYST  Active  An individual who has a Master's degree as a Board Certified Behavior Analyst and provides individualized service to assess and remediate identified behavioral challenges, which inhibit successful integration into the community. This individual may not provide cognitive therapies or psychological testing, neuropsychology, psychotherapy, sex therapy, psychoanalysis, hypnotherapy, and long-term counseling as treatment modalities. 
C9  BEHAVIORAL HEALTH AIDE/PRACTITIONER  Active  An individual having completed the requirements of the Community Health Aide Program at the Behavioral Health Aide I, II, III, or Behavioral Health Practitioner level as outlined by the Community Health Aide Program Certification Board. This individual provides prevention, early intervention, and aftercare mental health and substance abuse services. BHA/Ps work under indirect or general supervision of a licensed behavioral health provider or licensed mental health clinician as outlined in the federal Community Health Aide Program Certification Board Standards and Procedures. D1  BEHAVIORAL HEALTH NURSE PRACTITIONER  Active  An individual qualified by an approved postgraduate program in nursing and licensed by the practicing state, commonwealth or territory to practice professional behavioral health nursing including prescribing in the specialty. 
C5  BEHAVIORAL HEALTH STUDENT  Active  An individual who is enrolled in or a graduate from a United States accredited program for a 1) Doctoral degree in Psychology; 2) Master’s or Doctoral degree in Social Work; 3) Advanced Practice Registered Nurse (APRN) master’s degree with psychiatric nursing specialty or 4) COSTEP/summer extern. 
70  CARDIOLOGIST  Active  An individual having special knowledge and experience in the branch of medicine dealing with the heart, its functions, and its diseases. 
E1  CARDIOVASCULAR TECHNICIAN  Active  An individual who performs electrocardiograms (EKGs), stress testing, and Holter monitors. 
D9  CARDIOVASCULAR TECHNOLOGIST  Active  An individual who assists physicians in diagnosing and treating cardiac (heart) and peripheral vascular (blood vessel) ailments. Cardiovascular technologists may specialize in three areas of practice: invasive cardiology, echocardiography, and vascular technology. 
66  CASE MANAGERS  Active  A health care professional who is employed to manage and coordinate complex medical cases to ensure quality and efficient use of health care resources. 
B9  CHAPLAIN  Active  An individual qualified to receive referrals to serve patients within their scope of service and document the services rendered. 
69  CHIROPRACTOR  Active  An individual who employs the doctrine and dogma of chiropractic. The science and art of restoring or maintaining health, based on the theory that disease is caused by interference with nerve function, and employing manipulation of the body joints, especially of the spine, in seeking to restore normal nerve function. 
01  CLINIC RN  Active  An individual who is qualified by an approved postsecondary program or baccalaureate or higher degree in nursing and licensed by the state, commonwealth, or territory to practice professional nursing. 
67  CLINICAL PHARMACY SPECIALIST  Active  An individual who is certified by the IHS National Pharmacy Credentialing Committee as a Clinical Pharmacy Specialist (CPS), for those patient care visits falling under the CPS scope of practice. This code also includes patient care visits by pharmacists who are locally privileged by the Medical Staff to perform this level of care, but are not yet certified nationally by the Credentialing Committee.

C8  COMMUNITY HEALTH AIDE/PRACTITIONER  Active  An individual having completed the requirements of the Community Health Aide Program at the Community Health Aide I, II, III, IV or Community Health Practitioner level as outlined by the Community Health Aide Program Certification Board. This individual provides acute care, emergency care, follow-up care, and preventative services. 53  COMMUNITY HEALTH REPRESENTATIVE  Active  A tribal or native community-based, well-trained, medically guided health care provider, who may include traditional native concepts in his/her work and is funded with IHS-CHR appropriations.
