PEN AND INK CHANGE:
In section 3-7.13C(3), Item 16, delete the statement "(Please insert a line for inserting this total figure opposite Item 16. This will be corrected when new forms are printed)."
The change has been made on the report (Form PHS-2809) and will be reflected on the supply being distributed to the Areas in the current forms distribution period (Quarter ending March, 1960).
