Transmittal Notice 04-02

BACKGROUND:

This is to transmit a copy of the Indian Health Manual (IHM), Part 5, Chapter 24, "Forms."  This chapter establishes Indian Health Service (IHS) policy, procedures, responsibilities, and operating instructions for the control forms.  The IHS Forms Management Program provides efficient management to enable the IHS to operate economically through the proper use and control of forms.
