Health professionals can apply online for an IHS Loan Repayment Program (LRP) award. In addition to completing the application, you must submit all required
documentation (copy of license to practice, verification of employment and loan documentation) before your application will be reviewed for an IHS LRP award.
Application Deadline
The IHS LRP application cycle begins October 1 and runs through the Friday of the second full week of August. Applications are evaluated monthly beginning in
January, or as soon as funds become available.
Online Application
You will be asked to provide the following information when completing the online application:
- Personal Information (name, address, etc.)
- Educational and Professional Background
- Financial Information
- Certification that you have read the Affidavit/Sample Contract
Once you have reviewed your information, you will have the opportunity to upload the required supporting documentation, print a copy of your application for
your files and submit the application to the IHS LRP branch office.
Your required documentation can also be sent via email, fax or mail to the IHS LRP branch office.

Contact the LRP Office if you would like to request an Application Handbook containing information and instructions on the
IHS LRP.
Required Documentation
You MUST submit the following documentation in support of your application:
License to Practice
A photocopy of your current, non-temporary, full and unrestricted license to practice (issued by any state, Washington, DC or Puerto Rico). Students or new
graduates must submit their license as soon as it is available.
Employment Verification
Documentation of your employment with an Indian health facility, as applicable:
- IHS employment documentation or offer letter.
- Commissioned Corps orders.
- Tribal employment documentation or offer letter.
- Notification of Personnel Action (SF-50B) – For current federal employees, this form documents your service with the federal government.
Loan Documentation
A copy of all current statements related to the loans that you are submitting as part of your LRP application.
Priority Consideration
The following applicants MUST submit the appropriate documentation to receive priority consideration for an LRP award.
American Indians and Alaska Natives
The IHS LRP gives priority consideration to American Indian and Alaska Native applicants who are members of federally recognized Tribes. You must submit a copy of
an approved
Verification of Indian Preference for Employment in the Bureau of Indian Affairs and the Indian Health Service (Form BIA-4432)
form with your application documenting your membership status.
IHS Scholarship Program Recipients
The IHS LRP gives priority consideration to applications submitted by IHS Health Professions scholarship recipients who have completed their IHS Scholarship
Program service obligation. You must submit a copy of your completion letter documenting that you have fulfilled your service obligation.
