Application Template Overview
– Provides an overview and highlights specific sections of the Project Narrative, selected example of a Best Practice, Other Activities, and Key Contacts templates. (33:01 minutes)
Use these documents to prepare your application and ensure that you have included all required documents in your application kit.
Project Narrative Templates – the Project Narrative will consist of two parts: 1) the main Project Narrative template and 2) individualized Best Practice templates. Carefully read the instructions and review the entire template before starting to prepare your program’s Project Narrative.
SDPI FY 2015 Best Practice Templates - Part 2 – The table below provides a template for each Best Practice to report data on the Required and Other Key Measures. The following resources will assist in reporting on Required Key Measures:
FY 2014 Best Practice Addendum [PDF – 486KB] – Provides the most current information on the Required and Optional Key Measures, along with suggested ways to obtain the Measures.
Other Activities Template [PDF – 1MB] – Use this template for any other objectives and activities that do not fit
in your chosen Best Practice(s). Note: using the Other Activities template does not count as a Best Practice.
Key Contacts Form [PDF - 40KB] – This form is required as part of your application. Complete and save the form electronically, then attach it to your application package using the "Other Attachments Form."
* Note: Dates are tentative per DGM policy (120 days before budget cycle start date). Official due date will be available on GrantSolutions. Sub-grantees will need to check with their primary program on submission date and instructions.

APPLICATION FORMS FOR APPLICANTS WITH SUB-GRANTEES OR SUB-CONTRACTORS
These forms are provided only for planning purposes or for use by applicants with sub-grantees or sub-contractors. The change to GrantSolutions application kit includes the official versions of these forms that must be completed by all applicants.
ENVIRONMENTAL REVIEW AND DOCUMENTATION FORM (NEPA)
The Environmental Review and Documentation Form is required for any grant project that includes building, remodeling, excavation, purchasing modular structures, change of land use, and related project activities. If you are required to use this form, see the IHS Environmental Review Manual for detailed information. This manual contains the form along with instructions for its use.
