Description:
The TRIAD program stands for Taking Responsibility In Addressing Diabetes. The TRIAD program is a collaboration between the Oneida Community Health Center Diabetes Team/Medical Clinic [Physicians/Educators/Nurses], Health Promotion Department [Wellness Coaches], and Oneida Family Fitness Center [Fitness Specialists or Personal Trainers.] It is a 12 week exercise program where patients work individually or as a family with a personal trainer and wellness coach to increase physical activity, improve cardiometabolic health, and lost weight. Data is collected that includes pre/post fitness assessment [BMI, Waist to Hip Ratio, Aerobic Fitness, Strength, Flexibility, and Weight Loss], pre/post wellness/vision assessment, and pre/post hemoglobin A1C levels.
