Primary Health Care: The Health Center Program

Program Type: Resources - Information or materials that might help develop a program/project in a community but can not be defined as a Best Practice, Promising Practice, or Local Effort.

Grants are awarded to increase access to comprehensive primary and preventive health care and improve the health status of underserved and vulnerable populations in the area to be served. Individual health center grant mechanisms include: (1) Community Health Centers; (2) Migrant Health Centers; (3) Health Care for the Homeless; (4) Public Housing Primary Care Program; and (5) School Based Health Centers. Applications should be designed to improve the availability, accessibility and provision of primary health care services. Funds may not be used for inpatient services, or to make cash payments to intended recipients of services.
