All applicants must meet the following eligibility requirements (below) when applying for one of the three IHS scholarships.

Citizenship

You must be a citizen of the United States at the time you apply for an IHS scholarship. Permanent resident aliens and other noncitizens are not eligible to apply.

Documentation of American Indian/Alaska Native Membership or Descendent Status

You must submit a copy of an approved Verification of Indian Preference for Employment in the Bureau of Indian Affairs and the Indian Health Service (Form BIA-4432) form with your application. Use this form as follows:

  • American Indian: Category A — A member of a federally recognized Tribe, band or community.
  • American Indian: Category B — A descendent of a federally recognized Tribe, band or community (you must also provide copies of birth certificates for yourself and the parent/grandparent who is/are the enrolled member[s]).
  • Alaska Native: Category D — You must also provide at least one of the following forms of documentation: (1) Tribal Enrollment Card; (2) Certificate of Degree of Indian Blood (CDIB) with Tribal Enrollment Number (if applicable); (3) Other official documentation listing name, blood quantum, status (original enrollee, descendent, etc.); and (4) Tribal Enrollment Number, if applicable.

NOTE: State-recognized Tribal members, terminated Tribal members and descendents are only eligible for the Preparatory or Pre-Graduate Scholarship Programs.
