Description:This presentation will review the impact of clinical pharmacy services on diabetes management, including improvements in glycemic control, medication optimization, and patient engagement. It will also discuss how pharmacist-led interventions can increase timely access to care through proactive follow-up, education, and medication management support. In addition, it will explore how collaborative practice agreements [CPAs] enable pharmacists to work at the top of their license within primary care teams. Finally, the session will highlight practical ways to integrate pharmacists into chronic disease workflows to strengthen team-based care and improve outcomes.
Objectives and Outcomes: As a result of completing this training, the healthcare team will be able to:1.Compare national diabetes prevalence with the prevalence within the Native American/AlaskaNative population.2.Examine the importance and role of clinical pharmacists in preventing and treating diabetes.3.Develop a collaborative practice agreement to integrate clinical pharmacists into primary care for chronic disease management.

Tuition Policy:

Tuition must be paid in full 8 weeks prior to the start date of any course. Request for refunds must be received in writing at least two weeks before the course begins. For each refund request, there will be an administrative charge of $100. No refunds will be made to registrants who fail to attend a course. If IHS CDE program cancels a course, then 100% of the tuition will be refunded.
