Description:This webinar explores how Community Health Representatives can use their unique position as trusted community members to identify and address the social determinants of health that drive diabetes outcomes in tribal communities. Participants will gain a working understanding of the five SDOH domains — economic stability, education, healthcare access, neighborhood environment, and social context — and how each directly affects diabetes self-management and clinical outcomes. Through three real-world case studies and discussion, the session demonstrates practical CHR strategies for screening, connecting patients to resources, and communicating social needs to the care team. Attendees will leave with actionable tools, including guidance on the PRAPARE screening instrument and referral pathway development, along with a personal SDOH action plan to implement in their programs. Objectives and Outcomes: As a result of completing this training, the healthcare team will be able to:1.Identify social determinants of health [SDOH] as a priority in tribal diabetes care and their impact on diabetes self-management and health outcomes.2.Apply SDOH screening tools to identify social needs and connect patients to appropriate resources.3.Integrate SDOH findings into team-based diabetes care by documenting social needs, communicating them to the care team, and supporting culturally responsive care planning.

Tuition must be paid in full 8 weeks prior to the start date of any course. Request for refunds must be received in writing at least two weeks before the course begins. For each refund request, there will be an administrative charge of $100. No refunds will be made to registrants who fail to attend a course. If IHS CDE program cancels a course, then 100% of the tuition will be refunded.
