Activity Overview:

This course is an E-Learning course that is divided into two parts covering the chemo and the mechanical aspects of endodontic cleaning and shaping. Part 1 covers the selection and use of chemotherapeutic agents in endodontics. Part 2 covers the proper use of conventional stainless steel hand files and nickel titanium rotary files. The hand filing section explains and demonstrates crown down technique, step back technique, negotiating narrow canals, as well as blocked and ledged canals. The rotary section covers basic principles of all rotary files and the detailed use of the analytic K3 nickel titanium file system.

After review of this DVD and completion of the post-test the clinician will be able to:

1. Describe the principles and advantages of an ideal tapered endodontic preparation.
2. Perform a Crown Down Step Back endodontic preparation using Gates-Glidden burs, orifice shapers and standard hand files.
3. Negotiate a blocked and ledged canal.
4. Negotiate a narrow “calcified” canal.
5. Explain the basic similarities and differences between the main types of rotary file systems.
6. State the logic behind a crown down preparation using both hand and rotary files.
7. Demonstrate using the K3 rotary file system and how to safely and properly use it.
8. Apply various chemo agents used in endodontics and explain their safe and proper usage.

Activity Overview:

This course is an E-Learning, online based course that covers all aspects of an emergency endodontic visit. It is structured around the SOAP note and will walk you through developing a good patient note, performing diagnostic tests, formulating a pulpal and perapical diagnosis, and providing initial emergency treatment. At the end of the course you will be given an exam. A score of 80 or higher is needed before the CDE system can print out your course certificate.

Activity Overview:

Before final restoration and after complete cleaning and shaping, the root canal system must be obturated. This course covers the most common and best supported techniques available today. Various techniques presented include cold lateral obturation [standard and non-standard], as well as warm vertical obturation utilizing the System B heat carrier, and a gutta percha gun. Techniques, tips, and detailed visual examples including demonstrations and 3D animations are used to explain and demonstrated these techniques.

Today in America, addiction is a public health crisis. The opioid epidemic and substance use disorders more broadly have become one of the most common chronic illnesses in the U.S. In 2016, more than 115 Americans die from an opioid overdose. The misuse of and addiction to opioids—including prescription pain relievers, heroin, and synthetic opioids such as fentanyl—is a serious national crisis that affects public health as well as social and economic welfare.

In 2015, nearly 21 million Americans – more than the number of people who have all cancers combined – suffer from a substance use disorder involving alcohol or drugs – but only 1 in 10 receives treatment. One in 7 people in the U.S. is expected to develop a substance use disorder at some point in lives.

The Office of the Surgeon General released the first ever Surgeon General’s report on Substance Use and Addiction in November 2016. Facing Addiction in America: The Surgeon General’s Report on Alcohol, Drugs, and Health addresses alcohol, illicit drugs, and prescription drug misuse, with chapters dedicated to neurobiology, prevention, treatment, recovery, health systems integration and recommendations for the future. It provides an in-depth look at the science of substance use disorders and addiction, calls for a cultural shift in the way Americans talk about the issue, and recommends actions we can take to prevent and treat these conditions, and promote recovery.

In 2018, the Office of the Surgeon General released a public health advisory to urge more Americans to carry a lifesaving medication, naloxone, which can reverse the effects of an opioid overdose. Additional help and resources are available.

In 2018, The Surgeon General released Facing Addiction in America: Spotlight on Opioids to provide opioid-relevant information for the 2016 Surgeon General’s Report. The Surgeon General’s opioid postcard has 5 actions every person can take to stem the opioid epidemic. For more opioid resources, go to hhs.gov/opioids.

This one hour presentation, directly provided to new dentists [new to IHS or tribal dental programs] by Area Dental Officers or Dental Support Centers, gives an overview of the mission, structure, and personnel systems of the Indian Health Service. It also provides new dentists a list of available resources including the CDE program, specialty consultants, information on Enduring Challenges, levels of care, Federal Tort Claims, and much more information that will help the new dentist quickly adjust to life in the IHS. Dentists completing the orientation will be given a course number and completion code by the presenting dental leader.

Individual session learning objectives are available in the Challenges Program and will be given by each of the course instructors. Overall, upon completion of this meeting/course, participants should be able to:

1. Describe the organizational structure of the Indian Health Service;
2. List at least one standard of care for each dental discipline; and
3. Define at least two best practices to help prevent tort claims.

Participants should be dentists with 5 or fewer years of service in an IHS, tribal, or urban dental program. This means that some participants may be within 5 years of dental graduation, while others may have 20 or more years of private practice or military experience but less than 5 years in an IHS, tribal, or urban dental program. This diversity in overall experience will enrich all participants.

Please note that partial credit is not available for participants. Travel days are Monday and Friday and to receive full credit for participation, participants should plan to attend all Tuesday through Thursday sessions.

This meeting will be held at Four Points Nashville Airport, 800 Royal Parkway, in Nashville, Tennessee. This hotel is less than 1 mile from the Nashville International Airport, and free shuttle service is available by Four Points by Sheraton from 5:00 a.m. to 11:00 p.m. Rooms may be booked using the room block provided by e-mail by the course director, with the federal rate of $223/night. Concepts of Dental Clinic Management Part III was cancelled due to COVID-19. This one-hour webinar is designed for attendees of that course.

To participate, you must have been enrolled in Concepts III and you must have received the article by Lucas Bishop. Conference call information will be sent out by the course instructors.

Summary: This webinar is based on an article by Lucas Bishop titled “Breaking Down the Skills Gap: Tribal Knowledge”. Students have been reviewing the article and the group will discuss what kinds of Tribal/organizational knowledge is leaving their programs and how they can capture that knowledge, hopefully coupling the historical experts with the inexperienced members in their clinic staff.

1.   Identify who has the bulk of organizational knowledge and what specific expertise should be captured and passed on.

The opioid epidemic and substance use disorders more broadly have become one of the most common chronic illnesses in the U.S. In 2017, more than 130 people died daily from opioid-related drug overdoses. The misuse of and addiction to opioids—including prescription pain relievers, heroin, and synthetic opioids such as fentanyl—is a serious national crisis that affects public health as well as social and economic welfare. Medication-Assisted Treatment [MAT] is the use of medications in combination with counseling and behavioral therapies, to provide a “whole-patient” approach to the treatment of substance use disorders. This one-hour webinar will focus on understanding the medications used for Medication-Assisted Treatment for Opioid Use Disorder and how to manage dental pain in patients on MAT.

Dental assistants must have basic knowledge about periodontal disease before attending the Periodontal Expanded Functions-Basic course. The basic knowledge includes: what makes oral hygiene instruction effective; how to modify patient behavior to improve compliance with oral hygiene regimens; predisposing factors that relate to motivation to act or change; how antimicrobials work, and their use in preventing and treating periodontal diseases.

This online course, created in 2016 and updated annually, consists of pre-tests, learning activities, and post-tests in the two modules. To complete this course, dental assistants must earn a score of 80 percent or better on each post-test. Dental assistants must successfully complete these two modules prior to attending the Periodontal Expanded Functions-Basic course.

At the end of this course, participants will be able to:
• Distinguish between effective and ineffective oral hygiene instruction.
• Discuss methods about how to modify patient behavior to improve compliance with oral hygiene regimens.
• Clarify how antimicrobials work and their use in preventing and treating periodontal diseases.

Dental assistants must have a core set of knowledge and skills before registering for the Expanded Functions-Basic Restorative course. The core set of knowledge and skills include: dental terminology; placing and removing a Tofflemire matrix band and wedge, preparing, placing and removing a dental dam; and setting up for Class I, Class II, and Class V amalgams, and Class III and Class V composite restorations.

This online course consists of pre-tests, learning activities, post-tests and demonstrations for each lesson in the four modules. To complete this course, dental assistants must earn a score of 80 percent or better on each post-test, and certify competency by demonstrating skills and knowledge to their supervisor. Upon successful completion of all four modules in this course, dental assistants will be able to register for the EF-Basic Restorative Course.

Upon completion of this course, the participants will be able to:
•Identify parts and surfaces of teeth.
•Use universal numbering system.
•Define tooth anatomy terms.
•Describe the caries process and identify cavity classifications.

The purpose of this course is to introduce the dental assistant to the placement and finishing of restorative materials used in operative dentistry. Technical background information is presented in cavity classification and dental anatomy. Class I, II, & V amalgam restorations, and Class III & V composite restorations are covered in lecture and laboratory exercises. Other written and practical examinations are used to measure participant progress. The basic course has been organized for those dental assistants with limited or no training and/or experience in the placement of restorative materials. It is only practical for those clinics where the ratio of dentists to dental assistants allows the practice of expanded restorative functions. If the newly acquired skills are to become fully developed, the assistant must be able to return to his/her clinic and work with their dentist for a recommended period of 6 months. This follow-up supervised placement of amalgam and composite restorations is required in order to complete the course with a Certification as a Restorative Expanded Function Dental Assistant. Students will receive CDE credits for their training time during this class, without regard to grades or future certification.

1. Describe tooth morphology and structure and the materials used to restore them.
2. Place and finish basic amalgam and composite restorations.
3. Discriminate between what is an acceptable and an unacceptable restoration.

This course is a continuation of the “Basic Expanded Functions” course, which is a prerequisite.

All five-cavity classifications/preparations will be covered in lecture and laboratory exercises, including amalgam and composite restorations. Lectures will review and update course participants on dental restorative materials including a discussion of bonding agents. Laboratory exercises will be performed on typodonts beginning with conservative preparations and working up to and including cusp-protected amalgam and incisal angle composite resin restorations. Also, participants will be introduced to the basics of occlusion. The course will require that the participant perform 5 cusp-protecting amalgams [CPAs] and 5 class IV composite restorations back in their clinical setting before receiving her/his certificate.

The purpose of this course is to introduce the dental assistant to the placement and finishing of composite [and glass ionomer] restorative materials in IHS, tribal, and IHS-funded urban dental programs.

Course Overview: This course will combine concepts taught in the restorative EFDA basic and advanced courses but will focus on composite resin restorations. Background information will be provided in cavity classification and dental anatomy. Anterior composites will include both Class III and Class IV restorations taught in didactic and laboratory settings. Posterior composites will include Class I, Class II, and Class V restorations taught in didactic and laboratory settings. This one-week course is designed to provide basic skills to perform these restorations.
Prerequisites: The dental program director and dental assistant supervisor are responsible for ensuring the dental assistant is adequately prepared the course. Completion of the restorative EFDA basic and/or advanced courses [which teach amalgams and composites but exclude Class I and II composites] is not a prerequisite requirement to take the composite course. However, the prospective EFDA must complete DA0004, the Restorative Expanded Functions Basic Prerequisite Course, and this must be completed at least two weeks in advance of the course. If the dental assistant fails to meet this deadline, he/she will be unenrolled from the restorative EFDA composite course regardless of whether tuition has been paid or not and travel has been arranged or not. The Restorative Expanded Functions Basic Prerequisite Course consists of four online modules: [1] dental anatomy, [2] tofflemire matrix band and wedge [which isn’t used in composites but nevertheless teaches fundamentals], [3] dental dam, and [4] operative setup. It is also the dental program director’s and DA supervisor’s responsibility to provide some training prior to the course, specifically to help the DA understand the operative setup for composites at the clinic, instruments used for composites, etc.
Course Curriculum: The Restorative EFDA – Composite Course will consist of 32.0 hours of continuing dental education taught over five days by a skilled and calibrated instructor using a standard training curriculum as listed below. The dental assistant must attend all five days to receive CDE credit and to move on to the post-course portion of certification; no partial credit is given. In addition, the dental assistant must successfully pass both the written and practical tests. Failure to pass the tests, as determined by the instructor[s] and course director, will result in failure of the entire course and the dental assistant will need to remediate the course at a future date.

Post-Course Responsibilities: It is important to remember that this one-week course is designed to provide the basic information and skills for a dental assistant to successfully place anterior and posterior composite restorations. Since the dental assistants will work under the license of a supervising dentist, it is that dentist’s responsibility to ensure that the dental assistant is competent to provide the service to the patient. The supervising dentist incurs all responsibility of the services provided to patients by the dental assistant. Consequently, we recommend that the supervising dentist[s] continue to provide training of restorative expanded function dental assistants as needed. Similarly, as a measure of quality of care, we encourage all dental assistants successfully completing the Restorative EFDA – Composite Course to become certified [read below on this process]. Supervising dentists should carefully grade restorations using the quality criteria sent by the course director in the post-course letter.
Certification: Dental assistants must complete the following composite restorations within a six-month period in order to obtain EFDA certification: 40 total restorations, of which at least 10 are Class I or Class V restorations, at least 10 are Class III or Class IV restorations, and at least 10 are Class II restorations.
Once the dental assistant has successfully completed 40 restorations [graded as “A” or “B”], the dental assistant supervising dentist [not DA supervisor, but supervising dentist] should e-mail the IHS CDE Coordinator with the following statement:
“___________ [name of DA] has successfully completely 40 composite restorations that include at least 10 Class I or V restorations, at least 10 Class III or IV restorations, and at least 10 Class II restorations. ______ [name of DA] completed the Restorative EFDA – Composite Course held at _________ [location] on ___________ [dates]. A copy of the evaluation record will be maintained on file at this clinic. Please issue instructions for certification.”
The dental assistant’s supervising dentist[s] may consult with the DA’s instructor if they have questions about the quality evaluation criteria. However, it is the supervising dentist’s responsibility to ensure that the dental assistant successfully meets the certification criteria [again, the DA will be working under that dentist’s license in performing the procedures].

Upon completion of this course, participants will be able to:
1. Have an understanding of the basic concepts of occlusion as they apply to restorative dentistry;
2. Be able to place and finish composite restorations - Class I, II, III, IV, and V;
3. Have an understanding of composite materials, dental adhesives, curing lights, and cavity liners; and
4. Have the ability to discriminate between acceptable and unacceptable restorations.

Post-Course Responsibilities: It is important to remember that this one-week course is designed to provide the basic information and skills for a dental assistant to successfully place anterior and posterior composite restorations. Since the dental assistants will work under the license of a supervising dentist, it is that dentist’s responsibility to ensure that the dental assistant is competent to provide the service to the patient. The supervising dentist incurs all responsibility of the services provided to patients by the dental assistant. Consequently, we recommend that the supervising dentist[s] continue to provide training of restorative expanded function dental assistants as needed. Similarly, as a measure of quality of care, we encourage all dental assistants successfully completing the Restorative EFDA – Composite Course to become certified [read below on this process]. Supervising dentists should carefully grade restorations using the quality criteria sent by the course director in the post-course letter.
Certification: Dental assistants must complete the following composite restorations within a six-month period in order to obtain EFDA certification: 40 total restorations, of which at least 10 are Class I or Class V restorations, at least 10 are Class III or Class IV restorations, and at least 10 are Class II restorations.
Once the dental assistant has successfully completed 40 restorations [graded as “A” or “B”], the dental assistant supervising dentist [not DA supervisor, but supervising dentist] should e-mail the IHS CDE Coordinator with the following statement:

Post-Course Requirements: Students must complete 20 dental cleanings which are checked and scored by the hygienist/dentist at their home dental clinic. This is documented on the Follow-Up Training Progress Record, which is then maintained at the local program. The dental chief/director should then e-mail the IHS EFDA Coordinator [IHSEFDA@ihs.gov] to inform them that post-course requirements have been met and that the record is on file at the local clinic. This course is a continuation of the “Basic Expanded Functions” course, which is a prerequisite.

All five-cavity classifications/preparations will be covered in lecture and laboratory exercises, including amalgam and composite restorations. Lectures will review and update course participants on dental restorative materials including a discussion of bonding agents. Laboratory exercises will be performed on typodonts beginning with conservative preparations and working up to and including cusp-protected amalgam and incisal angle composite resin restorations. Also, participants will be introduced to the basics of occlusion. The course will require that the participant perform 5 cusp-protecting amalgams [CPAs] and 5 class IV composite restorations back in their clinical setting before receiving her/his certificate.

Dental assistants must have basic knowledge about periodontal disease before attending the Periodontal Expanded Functions-Basic course and must successfully complete the Prerequisite Course at least 2 weeks prior to attending the course.
