Today in America, addiction is a public health crisis. The opioid epidemic and substance use disorders more broadly have become one of the most common chronic illnesses in the U.S. In 2016, more than 115 Americans die from an opioid overdose. The misuse of and addiction to opioids—including prescription pain relievers, heroin, and synthetic opioids such as fentanyl—is a serious national crisis that affects public health as well as social and economic welfare.

In 2015, nearly 21 million Americans – more than the number of people who have all cancers combined – suffer from a substance use disorder involving alcohol or drugs – but only 1 in 10 receives treatment. One in 7 people in the U.S. is expected to develop a substance use disorder at some point in lives.

The Office of the Surgeon General released the first ever Surgeon General’s report on Substance Use and Addiction in November 2016. Facing Addiction in America: The Surgeon General’s Report on Alcohol, Drugs, and Health addresses alcohol, illicit drugs, and prescription drug misuse, with chapters dedicated to neurobiology, prevention, treatment, recovery, health systems integration and recommendations for the future. It provides an in-depth look at the science of substance use disorders and addiction, calls for a cultural shift in the way Americans talk about the issue, and recommends actions we can take to prevent and treat these conditions, and promote recovery.

In 2018, the Office of the Surgeon General released a public health advisory to urge more Americans to carry a lifesaving medication, naloxone, which can reverse the effects of an opioid overdose. Additional help and resources are available.

In 2018, The Surgeon General released Facing Addiction in America: Spotlight on Opioids to provide opioid-relevant information for the 2016 Surgeon General’s Report. The Surgeon General’s opioid postcard has 5 actions every person can take to stem the opioid epidemic. For more opioid resources, go to hhs.gov/opioids.

Dental assistants must have basic knowledge about periodontal disease before attending the Periodontal Expanded Functions-Basic course. The basic knowledge includes: what makes oral hygiene instruction effective; how to modify patient behavior to improve compliance with oral hygiene regimens; predisposing factors that relate to motivation to act or change; how antimicrobials work, and their use in preventing and treating periodontal diseases.

This online course, created in 2016 and updated annually, consists of pre-tests, learning activities, and post-tests in the two modules. To complete this course, dental assistants must earn a score of 80 percent or better on each post-test. Dental assistants must successfully complete these two modules prior to attending the Periodontal Expanded Functions-Basic course.

At the end of this course, participants will be able to:
• Distinguish between effective and ineffective oral hygiene instruction.
• Discuss methods about how to modify patient behavior to improve compliance with oral hygiene regimens.

Dental assistants must have a core set of knowledge and skills before registering for the Expanded Functions-Basic Restorative course. The core set of knowledge and skills include: dental terminology; placing and removing a Tofflemire matrix band and wedge, preparing, placing and removing a dental dam; and setting up for Class I, Class II, and Class V amalgams, and Class III and Class V composite restorations.

This online course consists of pre-tests, learning activities, post-tests and demonstrations for each lesson in the four modules. To complete this course, dental assistants must earn a score of 80 percent or better on each post-test, and certify competency by demonstrating skills and knowledge to their supervisor. Upon successful completion of all four modules in this course, dental assistants will be able to register for the EF-Basic Restorative Course.

Upon completion of this course, the participants will be able to:

The periodontal EFDA course is designed to teach dental assistants to provide dental cleanings to remove visible calculus by using state-of-the art Piezoelectric scalers or Magnetostrictive ultrasonic scalers. Hand scaling is no longer a part of the periodontal EFDA curriculum.

Pre-Course Requirements:

1. All DAs registered in this course must complete the prerequisite course, DA0001, at least 2 weeks prior to the start of the course. Failure to do so may result in removal from the course by the course director.
2. All DAs registered in this course should be prepared to take the course. If you feel that you need additional preparation prior to the course, please work with your dental director.


During the Course:
1. Participants are expected to pass a series of didactic [lecture] and clinical examinations to show proficiency in course content. This includes clinical work with typodonts and with real patients.
2. At the conclusion of the weeklong course, the course director will provide participants with a completion code. The DA will then have 3 days to complete the online post-course evaluation and get CDE credit. Failure to get CDE credit will result in non-certification down the road, so it is important to do this right away!

Post-Course Requirements:
1. DAs must complete 20 dental cleanings which are checked and scored by the hygienist/dentist at their home dental clinic. These should be done under direct supervision of the dentist or hygienist [dentist/hygienist in the room while the DA is performing the cleaning, able to provide tips and recommendations as needed]. 2. Post-course requirements must be completed within 6 months of the EFDA training.

When the DA has satisfactorily completed the post-course cleanings, the dental chief/director under whose license the DA will be working should then e-mail the IHS EFDA Coordinator [IHSEFDA@ihs.gov] to inform them that post-course requirements have been met and that the record is on file at the local clinic. 5. After the 20 cleanings done under direct supervision by the dentist/hygienist, the DA should NOT perform any further cleanings unless CERTIFIED by the IHS EFDA Coordinator. Following certification, the new EFDA may perform cleanings under indirect supervision [dentist/hygienist in the facility but not directly overseeing the work until a final check is needed].

Annual Competency:
1. The Dental Director/Dental Chief is encouraged to complete an annual competency evaluation on the periodontal EFDA to ensure high quality care is being delivered.
2. The annual competency evaluation can be found on the IHS Dental Portal under the Clinic Tab - Competencies.

Upon completion of this course, participants will be able to:
1. Relate Community Periodontal Index of Treatment Needs [CPITN] scores to a need for potential periodontal treatment.
2. Detect disease, supra and subgingival calculus.
3. Provide thorough ultrasonic scaling of teeth..
4. Accurately code for periodontal and hygiene procedures with IHS/ADA coding. .
5. Recommend effective toothpastes, mouth rinses and oral hygiene aids to patients.
6. Motivate patients to improve plaque removal and periodontal health.
7. Identify who is at risk for periodontal breakdown.

Dental assistants must have basic knowledge about periodontal disease before attending the Periodontal Expanded Functions-Basic course and must successfully complete the Prerequisite Course at least 2 weeks prior to attending the course.

The purpose of this course is to introduce the dental assistant to the placement and finishing of restorative materials used in operative dentistry. Technical background information is presented in cavity classification and dental anatomy. Class I, II, & V amalgam restorations, and Class III & V composite restorations are covered in lecture and laboratory exercises. Other written and practical examinations are used to measure participant progress. The basic course has been organized for those dental assistants with limited or no training and/or experience in the placement of restorative materials. It is only practical for those clinics where the ratio of dentists to dental assistants allows the practice of expanded restorative functions.

Pre-Course Requirements:

1. All DAs registered in this course must complete the prerequisite course, DA0004, at least 2 weeks prior to the start of the course. Failure to do so may result in removal from the course by the course director.

2. Place and finish basic amalgam and composite restorations.

The advanced course builds upon the restorative basic course. All five-cavity classifications/preparations will be covered in lecture and laboratory exercises, including amalgam and composite restorations. Lectures will review and update course participants on dental restorative materials including a discussion of bonding agents. Laboratory exercises will be performed on typodonts beginning with conservative preparations and working up to and including cusp-protected amalgam and incisal angle composite resin restorations. Also, participants will be introduced to the basics of occlusion.

Pre-Course Requirements:

1. All DAs registered in this course must have completed - and been certified [completed post-course requirements] - as a Restorative BASIC EFDA, and must have been practicing as a Restorative BASIC EFDA for at least one year prior to taking this course.
2. All DAs registered in this course should be prepared to take the course. If you feel that you need additional preparation prior to the course, please work with your dental director. You are NOT required to retake the Restorative Prerequisite Course to take this course, but you may do so if you would like to refresh your knowledge.
3. All DAs and course instructors participating in this course must comply fully with IHS COVID-19 vaccination requirements. Failure to comply will result in removal from the course by the course director.

During the Course:
1. Participants are expected to pass a series of didactic [lecture] and clinical examinations to show proficiency in course content. This does not include clinical work with real patients.

Following certification, the new Advanced EFDA may perform advanced restorations under indirect supervision [dentist in the facility but not directly overseeing the work until a final check is needed].

Annual Competency:
1. The Dental Director/Dental Chief is encouraged to complete an annual competency evaluation on the Restorative ADVANCED EFDA to ensure high quality care is being delivered.
2. The annual competency evaluation can be found on the IHS Dental Portal under the Clinic Tab - Competencies.

Upon completion of this course, participants will be able to:
1. Have an understanding of the basic concepts of occlusion as they apply to restorative dentistry.
2. Be able to place and finish complex Class II amalgam restorations and composite restorations involving the incisal angle.
3. Have an understanding of the current state-of-the-art in dentinal bonding agents.
4. Have the ability to discriminate between acceptable and unacceptable restorations

The purpose of this course is to introduce the dental assistant to the placement and finishing of composite [and glass ionomer] restorative materials in IHS, tribal, and IHS-funded urban dental programs.

Course Overview: This course will combine concepts taught in the restorative EFDA basic and advanced courses but will focus on composite resin restorations. Background information will be provided in cavity classification and dental anatomy. Anterior composites will include both Class III and Class IV restorations taught in didactic and laboratory settings. Posterior composites will include Class I, Class II, and Class V restorations taught in didactic and laboratory settings. This one-week course is designed to provide basic skills to perform these restorations.

Pre-Course Requirements:

1. All DAs registered in this course must have completed - and been certified [completed post-course requirements] - as a Restorative BASIC EFDA, and must have been practicing/certified as a Restorative BASIC EFDA for at least one year prior to taking this course. You do NOT have to be certified as a Restorative ADVANCED EFDA to take this course.
2. All DAs registered in this course should be prepared to take the course. If you feel that you need additional preparation prior to the course, please work with your dental director. You are NOT required to retake the Restorative Prerequisite Course to take this course, but you may do so if you would like to refresh your knowledge.
3. All DAs and course instructors participating in this course must comply fully with IHS COVID-19 vaccination requirements. Failure to comply will result in removal from the course by the course director.

During the Course:
1. Participants are expected to pass a series of didactic [lecture] and clinical examinations to show proficiency in course content. This does not include clinical work with real patients.
2. At the conclusion of the weeklong course, the course director will provide participants with a completion code. The DA will then have 3 days to complete the online post-course evaluation and get CDE credit. Failure to get CDE credit will result in non-certification down the road, so it is important to do this right away!

Post-Course Requirements:
1. DAs must complete 40 composite restorations as specified in the Progress Record which are checked and scored by the dentist at their home dental clinic. These should be done under direct supervision of the dentist [dentist in the room while the DA is performing the restoration, able to provide tips and recommendations as needed]. Grading is documented on the Follow-Up Training Progress Record provided by the course director, which is then maintained at the local program.
2. Post-course requirements must be completed within 6 months of the EFDA training.
3. When the DA has satisfactorily completed the post-course cleanings, the dental chief/director under whose license the DA will be working should then e-mail the IHS EFDA Coordinator [IHSEFDA@ihs.gov] to inform them that post-course requirements have been met and that the record is on file at the local clinic. This should be done using this format only: "_______________________ [name of DA] has satisfactorily completed the post-course requirements as a Restorative COMPOSITE-ONLY EFDA. He/she took course ____ [course number] on ______ [date] at ______________ [location]. A copy of his/her evaluation form is being maintained in this office. 4. Once this is done, the IHS EFDA Coordinator will provide the student with instructions on how to receive their CDE certificate and their EFDA certificate.

After the 10 restorations are done under direct supervision by the dentist, the DA should NOT perform any further composite restorations taught in this course unless CERTIFIED by the IHS EFDA Coordinator. Following certification, the new Composite-Only Restorative EFDA may perform advanced composite restorations under indirect supervision [dentist in the facility but not directly overseeing the work until a final check is needed].

Annual Competency:
1. The Dental Director/Dental Chief is encouraged to complete an annual competency evaluation on the Restorative Composite-Only EFDA to ensure high quality care is being delivered.
2. The annual competency evaluation can be found on the IHS Dental Portal under the Clinic Tab - Competencies.

Upon completion of this course, participants will be able to:
1. Have an understanding of the basic concepts of occlusion as they apply to restorative dentistry.
2. Be able to place and finish complex Class II amalgam restorations and composite restorations involving the incisal angle.
3. Have an understanding of the current state-of-the-art in dentinal bonding agents.
4. Have the ability to discriminate between acceptable and unacceptable restorations

The periodontal EFDA course is designed to teach dental assistants to provide dental cleanings to remove visible calculus by using state-of-the art Piezoelectric scalers or Magnetostrictive ultrasonic scalers. Hand scaling is no longer a part of the periodontal EFDA curriculum.

Pre-Course Requirements:

5. Recommend effective toothpastes, mouth rinses and oral hygiene aids to patients.
6. Motivate patients to improve plaque removal and periodontal health.
7. Identify who is at risk for periodontal breakdown.

Dental assistants must have basic knowledge about periodontal disease before attending the Periodontal Expanded Functions-Basic course and must successfully complete the Prerequisite Course at least 2 weeks prior to attending the course.

The purpose of this course is to introduce the dental assistant to the placement and finishing of restorative materials used in operative dentistry. Technical background information is presented in cavity classification and dental anatomy. Class I, II, & V amalgam restorations, and Class III & V composite restorations are covered in lecture and laboratory exercises. Other written and practical examinations are used to measure participant progress. The basic course has been organized for those dental assistants with limited or no training and/or experience in the placement of restorative materials. It is only practical for those clinics where the ratio of dentists to dental assistants allows the practice of expanded restorative functions.

The basic course has been organized for those dental assistants with limited or no training and/or experience in the placement of restorative materials. The advanced course builds upon the restorative basic course. All five-cavity classifications/preparations will be covered in lecture and laboratory exercises, including amalgam and composite restorations. Lectures will review and update course participants on dental restorative materials including a discussion of bonding agents. Laboratory exercises will be performed on typodonts beginning with conservative preparations and working up to and including cusp-protected amalgam and incisal angle composite resin restorations. Also, participants will be introduced to the basics of occlusion.

The purpose of this course is to introduce the dental assistant to the placement and finishing of composite [and glass ionomer] restorative materials in IHS, tribal, and IHS-funded urban dental programs.

Course Overview: This course will combine concepts taught in the restorative EFDA basic and advanced courses but will focus on composite resin restorations. Background information will be provided in cavity classification and dental anatomy. Anterior composites will include both Class III and Class IV restorations taught in didactic and laboratory settings. Posterior composites will include Class I, Class II, and Class V restorations taught in didactic and laboratory settings. This one-week course is designed to provide basic skills to perform these restorations.

Pre-Course Requirements:

All DAs registered in this course must have completed - and been certified [completed post-course requirements] - as a Restorative BASIC EFDA, and must have been practicing/certified as a Restorative BASIC EFDA for at least one year prior to taking this course. Course Overview: This course will combine concepts taught in the restorative EFDA basic and advanced courses but will focus on composite resin restorations. Background information will be provided in cavity classification and dental anatomy. Anterior composites will include both Class III and Class IV restorations taught in didactic and laboratory settings. Posterior composites will include Class I, Class II, and Class V restorations taught in didactic and laboratory settings. This one-week course is designed to provide basic skills to perform these restorations.

Pre-Course Requirements:

1. All DAs registered in this course must have completed - and been certified [completed post-course requirements] - as a Restorative BASIC EFDA, and must have been practicing/certified as a Restorative BASIC EFDA for at least one year prior to taking this course. You do NOT have to be certified as a Restorative ADVANCED EFDA to take this course.

The advanced course builds upon the restorative basic course. All five-cavity classifications/preparations will be covered in lecture and laboratory exercises, including amalgam and composite restorations. Lectures will review and update course participants on dental restorative materials including a discussion of bonding agents. Laboratory exercises will be performed on typodonts beginning with conservative preparations and working up to and including cusp-protected amalgam and incisal angle composite resin restorations. Also, participants will be introduced to the basics of occlusion.

According to PL 97-35, the initial certification of competency in Dental Radiology involves two components:
1. Written Evaluation: "A criterion-reference test designed to measure knowledge and functional capability essential for making diagnostically acceptable radiographic surveys with minimal risk to the patient and the operator."
2. Clinical Evaluation: "The written examination is supplemented as necessary with a clinical examination." Demonstration of clinical proficiency means meeting the minimum standards in Infection control, radiology protection, quality of radiographs and presentation of radiographs.

The radiology certification process is intended to be accomplished at the dental auxiliarys facility under the guidance of the supervising dentist.

This course is the online version of the written exam.
Upon successful completion [a grade of 75 percent or higher] on the online exam and the completion of the clinical portion of the radiology certification process, the dental auxiliary will receive a certificate of competency in radiology.

To complete the online clinical portion of the radiology exam process, the supervisor must enter the clinical evaluations online in the dental directory. Go to the Assistants listing in the directory and use the drop down menu under their name to select "Add Radiology Clinical Eval" which will present the evaluation entering screen.

To complete the online clinical portion of the radiology exam process, the supervisor must enter the clinical evaluations online in the dental directory. Go to the Assistants listing in the directory and use the drop down menu under their name to select "Add Radiology Clinical Eval" which will present the evaluation entering screen.

It is recommended that dental auxiliaries study for the exam using Digital Imaging course DA2001.

• Stand at least six feet from the patient and not in the path of the primary beam or behind protective barrier during exposure.
• Allow only necessary persons in radiographic area during exposures.
• Give a warning prior to pushing the X-ray activator button.
• Position receptor and cone correctly without being held by staff or patient during exposures.
• Wear dosimeter.

Take, Process, and Upload into Proper Template/ Store Diagnostic Dental Digital Images
• Place receptor appropriately for the area of the mouth under study.
• Take images with correct density and contrast such that anatomical hard and soft tissue landmarks can be differentiated.
• Take images with no distortion in the area of the mouth under study.
• Take images with no overlapping in the area of mouth under study, except where tooth alignment does not permit open contacts.
• Take images with no cone-cutting.
• Take bitewing images that show the distal surface of the erupted canine and distal surface of the second molar.
• Use appropriate template to assure dental images are dated, identified with patient’s name and chart number, and contained in the patient’s electronic dental record.

The HHS Office of Minority Health [OMH] developed a cultural competency [now called cultural humility] program for oral health professionals. Think Cultural Health includes three two-hour online modules for a total of six hours of continuing dental education. This program is built upon the National Standards on Culturally and Linguistically Appropriate Services in Health and Health Care [the National CLAS Standards]. The National CLAS Standards offer a framework for individuals working in health and health care on which to build policy, programs and services.

The three courses introduce you to the fundamentals of cultural and linguistic competency and its relevance to oral health care. The courses were designed to be taken sequentially. To receive continuing dental education credit for a specific course, you will need to complete it in its entirety and obtain a minimum passing score 70 on the posttest included at the end of the course. CDE is offered not through IHS [there is no corresponding IHS course], but through OMH. When you have completed and passed the posttest for all three courses, you will have the option of printing a printer-friendly version of the course content from the Resources section under the Toolkit.

This program introduces cultural and linguistic competency skills you can use in your day-to-day practice. These skills and concepts are reinforced through features such as:

· The "Filling in the Gaps" boxes designed to provide you with additional information, resources and applications of the course material;

· "Think About It" questions to encourage your reflection on the information;

There are many non-insulin medications for the treatment of hyperglycemia available to patients with type 2 diabetes. Recent studies suggest the benefit of new agents beyond blood sugar reduction. Mindful eating is an approach that focuses on teaching individuals physical cues of hunger and fullness, as well as enhancing meal satisfaction. This session will provide an overview of the success and use of mindful meditation and mindful approach to eating in diabetes self-management education. Traditional food ways have a long history of importance and use in health maintenance and disease prevention and treatment in AI/AN communities. Nutrition was traditionally learned experientially with stories, and engaging with foods through planting to food preparation to tasting. However, traditional ways are often underestimated as a part of health services. This session will include presentation of a variety of sensory learning methods that engage patients in learning how to:
Identify carbohydrates from non-carbohydrates
Distinguish whole grains from refined grains
Distinguish starchy vegetables from non-starchy vegetables
Increase the variety of fruits and vegetables eaten
Several tested and useful methods to teach patients about achieving nutrition balance, including innovative use of My Native Plate, My Diabetes Plate and other visual tools for diabetes care and self-management education and active learning will be shared.
