Summary:

Patient centered care is recognized as a critical part of providing quality health care to improve overall health and well-being. This session will provide an overview of what patient centered care is, what the benefits are in relation to diabetes care, and what techniques, tools, and partnerships are best for implementing a patient centered care model to enhance a patient’s ability to overcome common barriers of diabetes management and improve overall diabetes care outcomes.


Learning Objectives:

Upon completion of this course, participants will be able to:
1.   Identify common barriers and gaps in diabetes care to be able to fully assess the needs in their practice and /or community to formalize a plan of action to improve diabetes treatment and prevention outcomes.
2. Implement patient-centered strategies to engage, enable, and empower individuals to select sustainable interventions to enhance health and well-being and improve overall quality of life around diabetes management.
3. Utilize a patient-centered integrated team-based approach to coordinate services and augment patient-centered care within their clinical practice and throughout their community to support the patient’s ability to overcome common barriers of diabetes management and improve overall diabetes care outcomes.

Tuition Policy:

Tuition must be paid in full 8 weeks prior to the start date of any course. Request for refunds must be received in writing at least two weeks before the course begins. For each refund request, there will be an administrative charge of $100. No refunds will be made to registrants who fail to attend a course. If IHS CDE program cancels a course, then 100% of the tuition will be refunded.
