Summary:

This presentation will provide a snapshot of how the Community Health Representative Program and the Special Diabetes Program for Indians collaborate in the Albuquerque Area to provide diabetes treatment and prevention through CHR activities.

Learning Objectives:

Objectives and Outcomes: As a result of completing this training, the healthcare team will be able to:
1.Examine the IHS CHR Program through current programs and historical context.
2.Identify the crucial role of CHRs in Native communities, fostered by effective teamwork and communication.
3.Develop an interprofessional healthcare team, in collaboration with the CHR program, to provide improved health outcomes for the community.

Tuition Policy:

Tuition must be paid in full 8 weeks prior to the start date of any course. Request for refunds must be received in writing at least two weeks before the course begins. For each refund request, there will be an administrative charge of $100. No refunds will be made to registrants who fail to attend a course. If IHS CDE program cancels a course, then 100% of the tuition will be refunded.
