DESCRIPTION:Primary care case management integrates added supports to help patients reach goals within primary care and to coordinate services beyond it, such as specialty or community care. Health coaching is a chronic disease education model well suited to team-based primary care, emphasizing culturally appropriate, individualized education and self-management support. This presentation describes two Chinle IHS programs where health coaches use case management to expand patient support. For years, bilingual, bicultural health coaches have delivered diabetes education in primary care, focusing on high-risk patients.
