1. The objective of the Door2Door Diabetes Campaign at the Whiteriver Service Unit is to go door-to-door to educate about the danger and preventability of diabetes in the community. And then to provide a comfortable environment for patients to learn, and to connect local residents with needed medical appointments.


 
2. Over the course of two years, the Door 2 Door Diabetes Program staff visited every home on the White Mountain Apache Reservation and knocked on every door, and provided diabetes education, and blood sugar testing, or left a door hanger if no one was home.


 
3. In the first round of the diabetes Door2Door Campaign, 3406 homes were visited and education was provided in 1447 homes and blood sugars were checked for 881 people. One hundred and fifty (150) undiagnosed people need follow up blood work were identified and 238 referrals for CHR or clinic follow up were generated.


 
4. One benefit of the program is that hospital and clinic employees get out into the community and have an opportunity to see and appreciate their patients’ households and lives.


 
5. The Door 2 Door Diabetes Program utilizes a multidisciplinary team of federal and tribal staff and can be replicated in other communities.
