Summary: OPEN UNTIL FILLED. SALARY IS NEGOTIABLE BASED UPON EDUCATION LEVEL, EXPERIENCE, AND CREDENTIALS. Kima:w Medical is located in Hoopa, CA on the beautiful Trinity River. Hoopa is a small community of approximately 3,000 residents primarily Native American. We operate a medical clinic located on the Hoopa Valley Indian Reservation, partially funded by the Indian Health Service. The clinic is solely owned and operated by the Hoopa Valley Tribe. We are a Joint Commission accredited ambulatory clinic and behavioral health unit. Our primary programs and services are outpatient medical, dental, pharmacy, outreach services, senior nutrition, ambulance service, a grant funded diabetes department, and behavioral health unit [which includes mental health and substance abuse].

Registered Nurse, FT/Regular. Care/case management emphasis patient care coordinator for outpatient clinical services responsibility to assist in the overall management of high risk patients through chart review and health/behavioral assessments provides some coverage and relief to other RNs.
Duties: FUNCTIONS RESPONSIBILITIES: 1. Assess patient condition and establish appropriate nursing needs. Identifies goals and nursing actions required to meet diagnosis-specific goals. Develops a care plan that addresses physical, psychological, social, and rehabilitative discharge planning to meet those goals. 2. Collaborates with patient, family members, and health care team members with development, implementation, and evaluation of patient plan of care. Integrates cultural and age-specific differences when developing, implementing, and evaluating patient's plan of care. 3. Provides appropriate educational media for patients and/or family members that address areas of concern related to nursing diagnosis and self-care needs. 4. Provides nursing services to outpatient, and other emergency patients, including obstetrical emergencies. Follows nursing standards, policies, and procedures when delivering medical treatment to clients. Nursing treatment will include, but is not limited to safe handling of medications, IV medication administration, emergency procedures, including cardiac emergency etc. 5. Evaluates progress toward meeting patient care goals. Collaborates with patient, family members, and members of health care team to revise plan of care as needed. 6. Actively participates with orientation of new personnel. Serves as a positive role model for new employees while encouraging a high level of professional conduct and maintaining a positive work attitude. 7. Utilizes existing quality-improvement systems to create constructive change. Participates in development, identification, implementation, and evaluation of clinic and medical center goals. 8. Participates in economic utilization of supplies and assists in maintaining infection control policies while creating a safe working environment. 9. Maintains strict confidentiality with regards to patient information. 10. Utilizes health care team resources to maximize quality patient care. 11. Adheres to all policies and procedures of K'ima:w Medical Center. 12. Participation in the CQI process to ensure Joint Commission accreditation is maintained and on-going improvements in patient care outcomes. 13. Participate in clinic pharmacy procedures to include appropriate medication requisitions, stock maintenance, and removal of outdated or expiring medications. Employee participates in patient triage and patient case management. 14. Other duties assigned. SUPERVISORY RESPONSIBILITIES: May be required to act in a supervisory role during the temporary absence of the Nurse Manager. In IPC team, RN will provide supervision for MA. CMA. and LVN on daily basis.
Qualifications: Minimum Requirements: High school diploma or GED CA driver license CA RN License Bachelor degree preferred two years case management experience preferred.
