Tanana Chiefs Conference is now hiring for Lead RN Case Manager at the Chief Andrew Isaac Health Center--an award winning cultural super clinic located in Fairbanks--Interior Alaska's urban hub. The Fairbanks based clinic is itself a hub that serves the larger mission of supporting health and wellness to Indian Health Service beneficiaries--Alaska Natives and American Indians, as well as many non-beneficiaries, in 42 villages across the river systems of the Interior (Yukon, Koyukuk, Kuskokwim, Tanana and more). This 235,000 square mile region is the home of the Tanana Chiefs Conference. TCC is looking for a Lead RN Case Manager that has proven clinical, organizational & people skills and also embodies respect, humility and cultural sensitivity. The Lead RN Case Manager provides overall team and panel management to ensure care is optimized and coordinated from village or home community to tertiary care and back to home community. Position leverages population management tools and case management systems to provide excellent care coordination, chronic disease management, and a variety of enabling patient services. Duties: 1. Coordinates nursing schedule for specified team making sure that all providers on the team have nursing coverage. The Team RN is responsible for arranging last-minute coverage for their team providers when nursing staff is absent. 2. Takes and returns phone calls from patients, clinic staff, and outside agencies related to patient care including triage calls. Follows up on referrals and abnormal lab and x-ray results when needed. 3. Arranges patient appointments for specialized care outside of CAIHC. 4. Checks notifications at least twice daily. 5. Assists nursing staff when needed with screening patients, drawing blood, starting IVs, and administering medications. 6. Schedules patient appointments and responds to front desk needs involving patient requests. 7. Schedules nurse only visits for injections, BP checks, etc. 8. Supervises day-to-day team activity to ensure smooth workflow and facilitates weekly team staff meetings. 9. Approves/denies time off requests. Processes time cards and assesses each team nursing staff member in order to perform yearly evaluations. 10. Works with other Team RNs to ensure provider coverage on all teams. 11. Documents patient telephone encounters in the EHR. Also documents patient care coordination in the EHR. 12. Coordinates video teleconferences from villages and ANMC with team providers. 13. Keeps provider informed of pain medication refill requests from pain management patients. 14. Is responsible for notifying team patients of STD test results and treatment, if applicable, using standing orders. 15. Coordinates patient care with Team Case Manager and Team Care Coordinator. 16. Works closely with specific Department Manager to resolve staff issues, improve patient care, implement policies and procedures, and maintain open communication with nursing staff and providers. 17. Participates in Quality Assurance and Improvement activities. The Lead RN Case Manager provides overall team and panel management to ensure care is optimized and coordinated from village or home community to tertiary care and back to home community. Qualifications: Lead RN Case Manager Qualifications: 1. Must have a current Registered Nurse license for the State of Alaska; Bachelor of Science Degree in Nursing preferred. 2. Five years' experience as licensed RN preferred. 3. Two years' experience in positions of responsibility and independent decision making (Care Coordination, Charge Nurse, Nurse Manager). 4. Case management certification preferred. 5. Experience working in rural Alaska or providing health care to Alaska Natives preferred. 6. Three (3) years of practical case management experience, preferably in a rural Alaska based I.H.S. system preferred. 7. Must pass background check pursuant to federal Indian Child Protection and Family Violence Prevention Act requirement. 9. Knowledge of quality management and improvement processes for the evaluation and improvement of the quality of clinical services provided by the Primary Care Team. 10. Demonstrated experience in establishing and maintaining clinical and chronic disease databases.
