Summary: The Mandan, Hidatsa and Arikara Nation, also known as the Three Affiliated Tribes, is located on the Fort Berthold Indian Reservation in central North Dakota. The reservation is located on the Missouri River in McLean, Mountrail, Dunn, McKenzie, Mercer and Ward counties. The reservation consists of 988,000 acres, of which 457,837 acres are owned by Native Americans, either as individual allotments or communally by the tribe. The Tribal headquarters is located 4 miles west of New Town, ND. Chairman Mark N. POSITION SUMMARY: Under the direction of the Home Health Care Director, the staff RN manages and directs nursing care activities and coordinates care planning with other disciplines.
Duties: 1. Carries out the duties of the attending or discharging physician to insure quality patient care in the privacy of patients' home or place of residence; communicates patients' needs; makes referrals; and functions as a patient advocate to insure the coordination of all health care resources. 2. Delivers direct patient care; assesses patients' physical, psychosocial and/or emotional needs; provides patient and family teaching; triages patients and informs physician of patient status; responds to phone messages from patients and informs physicians as needed; assists patient in procuring home supplies. 3. Provide ongoing assessments of the patients; responsible for ensuring CMS guidelines for disciplines is adherence. 4. The RN is responsible for formulating/ensuring the physician-approved plan of care for each patient is completed in a timely manner. 5. Administers medications as assigned; serves as a resource person knowledge of medication uses, side effects, and dosage; checks for proper medication order; documents medications; obtains required consent forms to administer immunizations; notifies pharmacies of physician approved prescriptions; reviews drug samples to insure they have not exceeded the expiration date; labels sample drugs and educates patients of medication administration. 6. Supervise the clinical nursing home visits, schedules, and charting to ensure the plan of care is carried out as prescribed. 7. Provide health education to patients' and their family members. 8. Protect confidential information and clients' rights; participate in quality improvement activities. 9. Conducts performance evaluations, disciplinary actions and all other personnel matters. 10. Represent MHA Nation Home Health Care in a highly professional manner. 11. Strive to educate the community of the mission, vision, and values of Home Health in a positive approach to assist in the growth of the organization and assist the community in its health care needs. 12. Qualifications: SUPERVISORY REQUIREMENTS: • The requirements for managing others does exist. KNOWLEDGE, SKILLS, AND ABILITIES: Knowledge: • Knowledge of HIPAA and confidentiality regulations; local, state, and federal regulations, governing home health care provision. • Knowledge of MHA culture/demographics on the MHA geographical dynamics • Knowledge of community resources and organizations • Knowledge and experience of MS Office, (Word, Excel, etc.) Skills • Communication skills • Computer skills of electronic health record, Microsoft Office • Documentation skills • Professional and customer service skills Abilities • Ability to maintain confidentiality • Ability to maintain a professional manner at all times • Ability to respond to emergency situations MINIMUM QUALIFICATIONS AND EDUCATION: • Associates of Nursing (RN) from an accredited nursing program with five years of nursing experience, BSN preferred. • Current RN (Registered Nurse) licensure in the state of North Dakota
