Summary: Winslow Indian Health Care Center [WIHCC] is a Tribal 638 Program located in the north central Arizona along the Interstate-40 highway in Winslow, Arizona. WIHCC serves the healthcare needs of the Native American Indians in the southwestern portion of the Navajo Nation involving the communities of Winslow, Dilkon, Leupp, Tolani Lake, Teesto, Seba Dalkai, Indian Wells, Jeddito, and Whitecone.

WIHCC is an accredited Medical Home site with AAAHC. Under supervision of the Patient Centered Medical Home Manager, the Clinical Care Coordinator will work independently, in conjunction with multidisciplinary teams members involved in the care of patients at the Winslow Indian Health Care Center. The Clinical Care Coordinator will be responsible for the delivery of appropraite, timely, and beneficial care for patients by promoting quality and cost-effective health care outcomes in a seamless continuum of care. This is will be accomplished by working wthin established standards of clinical care coordination, case management principles, and advance access principles. Maintains confidentiality of all privileged information.
Duties: Maintains regular attendance and punctuality Dependable in attendance and job performance. Develops and maintains internal patient care database for continuous tracking and follow up for continuity of care for active management of patients assigned ? Collaborates proactively with all interdisciplinary team members and with a customer-focus to facilitate and maximize patient healthcare outcomes Coordinates multi-disciplinary patient care conferences for high risk or complex customers as needed ? Maintains compliance with applicable laws, such as patient confidentiality, abuse reporting, principles of consent and advanced medical directives ? Advocates for the customer/family at the service-delivery level and at the policy making level fostering the patient's/family's decision making, independence, and growth and development ? Utilized best practice model to identify, incorporate or develop best practices for panel management ? Collaborates with others to share and establish best practice for clinic and division ? Manages panel acute care and chronic care needs as well as health mainten ance, meeting or exceeding clinic expectations for health maintenance standards [GPRA] ? Integrates factors related to quality, safety, efficiency, and cost effectiveness in planning, delivering, monitoring and evaluating customer care by promoting the most effective and efficient use of human and financial resources ? Utilizes campus software packages and applications for ordering, scheduling, and tracking customer care ? Shares and receives information, opinions, concerns, and feedback in a supportive manner ? Works collaboratively by building bridges, and creating rapport with team members within departments and across the organization ? Assists in coordinating and tracking referrals in conjunction with WIHCC providers, specialists and close collaboration with the Inpatient and Outpatient RN Case Managers in CHS ? Functions as liaison for patients and providers by monitoring scheduled diagnostic studies, clinic appointments and procedures, such as CTs, MRIs, and other specialized testing ensures travel and quarters are arranged as needed for patients. Acts as the resource nurse in the clinic coordinates with other services lines as needed Initiates needed therapeutic measures and other general nursing care Participates in department, WIHCC teams and committees Active member of clinical team, contributing to overall team function, team building and enhancement Participates in the continued development of the role of case management and care coordination in the primary care advanced access model Coaches and mentors all clinical team members to build or enhance capability and competency Assist in the implementation of nursing system that supports the mission of the American Academy of Ambulatory Care Nursing [AAACN] Responds to emergency situations Makes meaningful improvement to services, programs, and processes and/or organizational effectiveness that create new value for customers and employees Knowledge of clinical quality improvement models and processes Uphold all principles of confidentiality and patient care to the fullest extent. Adhere to all professional and ethical behavior standards of the healthcare industry. Interact in an honest, trustworthy and dependable manner with patients, employees and vendors. Possess cultural awareness and sensitivity. Performs other duties as assigned.
Qualifications: Bachelor's degree or higher in Nursing required, but will consider Associate Degree Nursing with BSN completion within 4 years from date of hire and with two years of clinical nursing experience at WIHCC or two years as an RN supervisor or management experience. Completion of an accredited nursing program required, possess an unrestricted Registered Nursing license from any of the 50 states Previous case management experience and/or certification in case management [CCM]. BLS certification required Valid unrestricted and insurable driver's license
