1.  GENERAL INFORMATION
 
    a)  Print all notes/readme files.
 
    b)  It is recommended that the terminal output during the
        installation be captured using an auxport printer attached
        to the terminal at which you are performing the software
        installation.  This will ensure a printed audit trail
        if any problems should arise.

    c)  The patch fixes/modifies the following functionality:

        - Technical modifications:
	- added an AF2 xref to .01 field of MHSS Record
        - disabled LAYGO on PLACEMENT DISPOSITION in MHSS Record
        - removed screen from .17 field of MHSS Record
        - added a xref to the VISIT field for use in visit merging
        - removed * from REFERRED BY field in MHSS INTAKE
        - Fields in MHSS SUICIDE FORMS are no longer marked as Required
        - fixed mis-spellings:
  	  problem code 19  Personality Disorder
 	  Suicide Form drug types: Tylenol (Acetaminophen) and Tricyclic
          Antidepressant (TCA)
        - Adds a field Time Spent in Group to each group encounter record.
        - Added time spent in group to form print
        - Fixed the display on the suicide standard report
        - Modified suicide form entry to allow exiting w/o completing form
        - Removed the Staging Tool from the suppressed form
        - Converted the OTHER Provider 2 field to a free text entry and
          modified reports and outputs accordingly.
        - Modified the health summary to display Behavioral Health rather than
          MHSS in patient education display
        - Added several more signature lines to the treatment plan.
        - Modified display of main PDE screen.
        - Added a report that dumps the suicide data in a delimited format.
        - Added AXIS III to the POV screen for Regular, A/SA and Intake type
          of visits.
        - Added AXIS III to the form print if it exists.
        - Added number served to the administrative record entry
        - Added arrival time as a prompt on all screens.  Removed arrival
          time as a pop up window
        - Added CDMIS data elements: CDMIS COMPONENT TYPE to A/SA visit type
        - Added TARGET to prevention activity screen
        - Added new Type of Contact:  INTENSIVE OUTPATIENT
        - Added new activity code for CLINICAL SUPERVISION RECEIVED (66)
        - Changed activity code 32 to read CLINICAL SUPERVISION PROVIDED-
          PATIENT NOT PRESENT
        - Added 3 new problem codes:
          14.1  SCREENING FOR DEPRESSION  icd9:   V79.0 
          29.1  SCREENING FOR ALCOHOLISM  icd9:   V79.1
          38.2  MED REFILL - ISSUE OF REPEAT PRESCRIPTION  icd9:  V68.1

    In all UCI's running IHS Behavioral Health version 3.0:
 
    a) Save off all AMH* routines as a backup.

    c) This is a kids install.  The Kids file name is amh_0300.01k.
