1.  GENERAL INFORMATION
 
    a)  Print all notes/readme files.
 
    b)  It is recommended that the terminal output during the
        installation be captured.  This will insure a printed
        audit trail if any problems should arise.
    
    c) This patch, patch 5, includes the following modifications:
       - New Option:  Merge 2 visits on different dates. APCDVMDD
       - New Option:  Visit Display in Non-Browser mode. APCDVDP
       - New Option:  Complete Orphaned Radiology Visits. APCDKRV
       - New Option:  Complete Orphaned Pharmacy Visits.  APCDKPV
       - New Option:  Re-print Group Forms.
       - New Mnemonics:  AST   capture asthma related data.
                         HAST  capture historical asthma related data.
                         COC   coded chief complaint.  Capture Chief
                               Complaint in icd diagnosis format.
                         PCF   documents that this pcc visit was created
                               By PCC+.
                         PF    Peak Flow measurement type.
	                       O2 O2 Saturation measurement type.
                         PA    Pain measurement type.
       - 3m Coder:  modification made to support the interface with the 3M
         coder.  (APCD3M, APCD3ME)
       - stuff visit unique ID in visit record.  (APCDALV)
       - Sentinel Surveillance link from V Lab added.  (APCDALVR)
       - Prevent Undef in several places where a patient pointer may be null.  
         (APCDPRB, APCDDVD1
       - Allow lookup of inactive patients in visit display and visit list in
         a date range options.  (APCDVLST, APCDDISP)
       - Set a data entry date in the visit file for the data warehouse export.
         (APCDEA2, APCDEAP, APCDEGP0, APCDEHI2, APCDPE2, APCDDMUP)
       - DM update option:  add exam results, add education provider (APCDDMU1,
         ACPDDMU2)
       - Allow the PPPV report in the Visit Review Report to be run for just
         pharmacy, radiology, lab or immunization visits. (APCDDVD, APCDR00)
       - Print CPT codes on the PCC form print.  (APCDEF, APCDEFC, APCDEFP,
         APCDEFP1)
       - Allow re-printing of group forms.
       - Have user okay input before proceeding with group form entry.

       - Print VCN in WHAT mnemonic. (APCDEWHA)
       - Print operators name on Listing of Uncoded Diagnoses and Procedures
         Reports (APCDFPPV)
       - Fixed HRN display on QA Coding reports. (APCDFQAP)
       - Modified Complete Orphaned Reports and Purging options to allow
         Pharmacy and Radiology in addition to the existing Complete Orphaned
         Lab options.  (APCDKLV, APCDKLVR, APCDKLVP)
       - Fixed formatting on immunization display.  (APCDLIM)
       - Modified problem list entry option to use ^TMP rather than a local
         array due to PGMOV errors.  (APCDPL, APCDPL1, APCDPL2)
       - Fixed ECODE error in VRR.  (APCDR07)
       - Modified APCDVLK to use the AA index rather than the AC to speed up
         visit lookup.  (APCDVLK)
       - Set .37 field of visit (merged to) for billing in VISIT MERGE.
         (APCDVMRG)
       - Modified several input templates for Fileman 22.
       - Added V BLOOD BANK to items to be moved from one visit to another.
         (APCDKUL)

      Patch 4 included the following modifications:
       - modifies the immunization display when using the IM mnemonic.
       - checks for the required admitting diagnosis in the VRR for
         hospitalizations.
       - fixes the form print for operations.
       - adds a new data entry option for entering data for a cohort of
         patients.
       - adds a new mnemonic for entering provider driven overrides
         of health maintenance reminders
       - add mnemonics ECO2 and ECO3 for entering a second and third E-code
         for a POV.
       - adds the Place of Injury prompt to ECOD mnemonic
       - adds Level of Decision Making field to PHN mnemonic
       - adds screens to tobacco and tb factors in dm update screen
       - Added BP to DM Update screenman screen.
       - sends out all data entry templates with required changes for FM 22.
         They work now under either FM 22 or FM 21.
       - adds a new way to enter patient education topics.  The user can now
         enter a topic using an icd9 diagnosis code and 1 of 17 categories.
       - adds HSIG, HBE, HCOL for capturing historical Flex Sig, Barium 
         Enenma and Colonoscopy

     Patch 3 did the following:
       - Modifies the 3M coder interface.  (APCD3M,APCD3MG)
       - Adds a new data entry option for updating historical Diabetes
         information.  This option utilizes a screenman screen.  (APCDDMU*)
       - Adds a new data entry option to print a complete PCC visit in Encounter
         form format.  This can be used by users who do their data entry without
         filling out a form. (APCDEF,APCDEFC,APCDEFC1,APCDEFP,APCDEFP1,APCDEFU)
       - Added a field to the Refusals to allow an indication of whether 
         the refusal was because the item was not medically indicated.
       - Added NMI mnemonic to enter Refusals that were Not Medically Indicated.
       - Added a mnemonic to pick up Chief Complaint
       - Added a AOP mnemonic.  When used to enter procedures, this will prompt
         for anesthesia information.
       - Added a prompt for ordering provider to RAD mnemonic.
       - Added 2 cpt modifier fields to the Radiology templates.
       - Added VCN, if entered to the visit selection display (APCDALV,
         APCDALV1,APCDEWHA,APCDVLK).
       - In the option to move V File items from one visit to another, added
         the ability to move all of one V File type by ordering provider.

       - Fixed UNDEF in forms tracking report.  (APCDFOS1)

     Patch 2 did the following:
       - Added all visit types to the Default Visit Type field in the PCC
         Control File.
       - Added all visit types to the Default Visit Type field of the PCC
         Data Entry Site Parameters file.
       - Added a new DD call PCC Data Entry Coder Errors for future use.
       - Modified the following routines for PCC File 200 conversion:
         APCDAAC1,APCDEAPC,APCDEGP0,APCDEGPP,APCDFOS1,APCDGHVD,APCDR00,
         APCDR06, APCDRV,APCDVCHK
         APCD20P2 - post init to patch, adds new mnemonics
         APCDALVR - added BLRLINK to new command
         APCDAUTL - changed APCLDF call
         APCDDVC  - exclude Event visits from report
         APCDEA   - commented out date echo
         APCDEA2  - fixed Tran Code Tracking call
         APCDEAP  - fixed Tran Code Tracking call
         APCDEDTC - prevent undef
         APCDEH1  - prevent undef
         APCDEHS1 - new routine for Health Summary display in non-browser mode
         APCDEWHA - reduced lab display in WHAT mnemonic

         APCDPLK  - allow a 0 problem number
         APCDRVH1 - modified edit on # of dxs exported
         APCDVCHK - added a warning for ER mnemonic
         APCDVLST - added pov's to display
    New mnemonics:
         GHS - to display health summary in non-browser mode.
         REF - to enter a patient's refusal of service.

    WARNING:  it has been suggested that the PCPT mnemonic no longer
    be used because it may erroneously map CPT codes with Operation
    codes.  To disable this mnemonic, use the option to do so that is 
    under the Data Entry Supervisor menu.

3.  REQUIREMENTS
 
    a)  Kernel v 8.0 or later
 
    b)  FileMan v 21

    c)  AUT 98.10 PATCH 11 must be installed

    d)  AUPN Version 99.1 patch 7 must be installed.
 
    e)  APCD Version 2.0 patch 3 and patch 4 should be installed prior to 
        installing patch 5.

    f)  AUM version 03.1 through patch 01.

4.  INSTALLATION INSTRUCTIONS
 
    In all UCI's running PCC Data Entry:
 
    a)  This is a KIDS install.  Load distribution apcd0200.05k.
        After it is loaded, then install it.  If you need assistance
        With a KIDS install please call your Area Support personnel.
