GENERAL INFORMATION a) Print all notes/readme files. b) It is recommended that the terminal output during the installation be captured using an auxport printer attached to the terminal at which you are performing the software installation. This will ensure a printed audit trail if any problems should arise. ** IMPORTANT NOTE c) This is health summary patch 8. Patch 1 requires that pharmacy 6.0 be loaded. If you are still running a version of Pharmacy less than 6.0 you must save off routine APCHS7, load this patch, then restore routine APCHS7. When you load Pharmacy 6.0, you must reload this health summary patch. d) The patch fixes/modifies the following functionality: - modifies/upgrades all health maintenance reminders (see accompanying document) - fixes pap display on DM Supplement - fixes lab results to not display ^^ on DM Supplement - fixes potential undef in Medicare display - expand the lookup for paps, flu shot and mammograms on the supplement to include CPT codes - modifies the PDD prompt to state "Last Documented PPD" on supplement - modifies the prompt to state "Known Positive PDD or Hx of TB" on supplement - adds the date of the last Mammogram from PCC to supplement - adds a component to display allergies/adverse reactions from the pharmacy allergy tracking module Previous patches did the following: - modifies the allergy section to include new 2000 icd9 codes (APCHS40) - adds an entry point for printing health summaries from PCC Encounter Form system (APCHS,APCHS0) - modifies the Medications w/# issued component - adds a second hemoglobin A1C to the DM supplement - adds refusals for lab tests to the DM supplement - Modifies the education display (APCHS10) - Prevents a possible undef in Medicaid display (APCHS5A) - Adds return to stock note on Medication display (APCHS7) - Fixes possible PGMOV in dm supplement (APCHS9B4) - Adds new component which displays all outpatient encounters minus CHR visits. (APCHS2H) - Modifies the DM supplement per PSG. (APCHS9B1,APCHS9B2, APCHS9B4,APCHS9B5) - Adds type of refusal to the refusal component. (APCHS5) - Adds a PEP to SELPT^APCHS (APCHS) - Fixes CMS Display to coincide with CMS (APCHS) - Adds patient refusal checks to several health maintenance reminders. (APCHS11, APCHS11A, APCHS11C, APCHS11D, APCHS11E, APCHS11F, APCHS11G) - Checks for null provider narrative to prevent subscript errors. (APCHS40) - Adds 2 new Elder Care components (these are still being reviewed by PSG but are distributed to not overwrite the test sites. (APCHS5) - Significantly modifies the logic of the Diabetes Patient Care summary per the Diabetes group. (APCHS9B1, APCHS9B2, APCHS9B3, APCHS9B4) - Adds a new, modified lab component to display labs by Date (APCHS3B) - Adds a new, modified medication component. (APCHS73) 2. CONTENTS OF DISTRIBUTION a) apch0200.08k - Routines b) apch0200.08n - Installation notes c) apch0200.08b - globals file d) apch0200.08o - (.pdf format) Supplemental Health Summary document that describes the new health maintenance reminders. This is in pdf format and should be shared with the providers and used to update the health summary health maintenance reminder section of the health summary. 3. REQUIREMENTS a) Health Summary Version 2.0 (APCH) b) This is health summary patch 8. Patch 1 requires that pharmacy 6.0 be loaded. If you are still running a version of Pharmacy less than 6.0 you must save off routine APCHS7, load this patch, then restore routine APCHS7. When you load Pharmacy 6.0, you must reload this health summary patch. 4. INSTALLATION INSTRUCTIONS In all UCI's running Health Summary Version 2 a) Disable the Health Summary Menus to prevent users logging on to Health Summary b) Save off all APCH* routines as a backup. ****NOTE** SKIP items c and d and f if you have already loaded this patch once c) Do a Global Save of global APCHSURV. d) Delete or Kill global ^APCHSURV. e) This is a kids install. The Kids file name is apch0200.08k. f) Do a global restore from file apch0200.08b. **It is important that the health maintenance reminder section of each health summary that is in use at the facility be reviewed. Many health maintenance reminders have been deleted and a few have been added. The clinical staff should decide which reminders should be included in each health summary type. ** NOTE: 2 lab taxonomies need to be set up for use with the new health maintenance reminders: APCH FECAL OCCULT BLOOD - should contain all FOBT tests APCH HCT/HGB TESTS - should contain all lab tests that would indicate that a HCT/HGB was done. These taxonomies can be created using the Taxonomy Setup option in PCC Management Reports under the Diabetes QA Audit Menu.
