1.  GENERAL INFORMATION 
 
         a)  Print all notes/readme files.  
         b)  It is recommended that the terminal output during the 
             installation be captured using an auxport printer 
             attached to the terminal at which you are performing 
             the software installation.  This will insure a printed 
             audit trail if any problems should arise.
         c)  This patch corrects the following problems:
             1.  Prompts for subjective and objective data in the
                 screenman data entry screens.
             2.  Adds subjective and objective data to the form print.
             3.  Fixes several displays in the health summary.
             4.  Fixes the export to not error out if the first run
                 is done and data exists prior to the indicated date
                 range.  
 
2.  CONTENTS OF DISTRIBUTION 
 
         a)  bch_0100.02p  - patch 2 routine 
         b)  bch_0100.02n  - notes file for patch 2

3.  REQUIREMENTS 
 
        ****This patch should only be installed AFTER you have
		 installed the 1997 ICD Update********

4.  INSTALLATION INSTRUCTIONS 
 
         a.  Do a routine restore (^%RR) from file bch_0100.02p
         b.  D ^BCH1INIT
             answer yes to all questions, after this is done you
             may delete routines BCH1I*

         IF YOU HAVE NOT ALREADY INSTALLED PATCH 1, DO THE FOLLOWING:
	   c.  Run routine BCH10P1  (D ^BCH10P1)
 	   d.  You may now delete routine BCH10P1
