1. GENERAL INFORMATION
	
   This patch fixes the following:  

   1.Key Changes for Version 8.0 Patch 2

Key enhancements and revisions include the following: 

* New Executive Order (EO) Quality Transparency Measures Report:  Added this
 new report that is needed to meet the Federal mandates of transparency of
  quality measurement report as per Executive Order 13410.  This report
 includes a hard-coded option that uses preset report parameters that are
 the same as the National GPRA Report (e.g. report period is hard coded
 to July 1, 2007 – June 30, 2008) and a user-defined option where the user
 provides all report parameters.

* Other National Measures Report:  (1) Added functionality for user to run
 the hard-coded report or the user-defined report.  The hard-coded option
 uses preset report parameters that are the same as the National GPRA
 Report (e.g. report period is hard coded to July 1, 2007 – June 30, 2008) and 
 a user-defined option where the user provides all report parameters.
 (2) Added the Tobacco Cessation topic to this report that includes new,
 developmental GPRA logic.  This logic is more effective at identifying a
 current tobacco user and will be analyzed and may become the GPRA logic
 for 2009 reporting.  (3) Added third export file that is produced when
 the Area ONM Report is run.

* New functionality for excluding demo/test patients from all CRS reports:
  (1) Added option for creating and editing a search template of demo/test 
 patients.  (2) Added site parameter for excluding from all CRS reports demo/test
 patients that are included in the specified demo/test patients search template.

* Comprehensive National GPRA Export for 2007-2008:  Updated CRS 2007 Version
 7.0 (not CRS 2008 Version 8.0), National GPRA Reports menu by adding ,menu
  option “XP8” that will be used for creating a comprehensive export of National
 GPRA Report data for GPRA years 2007 and 2008.  This data will be based on
 logic contained in CRS 2007 Version 7.0.  This data is in support of the
 quality improvement initiative IHS is participating in along with colleagues
 from Harvard Medical School.

* Updated CRS Graphical User Interface:  The GUI contains all functionality
 included in CRS 2008 Version 8.0, including the updated CRS 2007 Version 7.0
 for the Comprehensive National GPRA Export for 2007-2008.

Logic changes to GPRA measures reported to Congress include: 

* Diabetes: Glycemic Control:  Added CPT codes 3044F and 3045F to documented
 A1c definition and CPT 3044F to Ideal Control definition.

* Diabetes: Blood Pressure Control: (1) For BP documented definition,
 expanded CPT II range from 3077F, 3080F to 3074F-3080F for BP documented
 definition and added logic requiring CPT codes to be documented on non-ER
 visit.  (2) For Controlled BP definition, added new logic for CPT II codes,
 which are required to be documented on non-ER visits.

* Adult Immunizations: Influenza:  Added CPT codes 90661 and 90662 to
 influenza definition.

* Childhood Immunizations:  (1) Added CPT 90696 to DTaP and IPV definitions.
  (2) Added text clarifying logic only requires 1 contraindication for 
 immunizations that require multiple doses (vs. requiring 1 contraindication
 for each dose).

* Colorectal Cancer Screening:  Revised numerator descriptions that use
 FOBT to include FIT (fecal immunochemical test) as well.  There are no
 separate codes for this test but the text was revised so sites know
 they can include this test in their site-populated taxonomy for occult blood.

* Tobacco Cessation: (1) Added CPT codes 99406 and 99407 to tobacco users
 and tobacco cessation counseling definitions. (2) Removed ADA code 1320
 from tobacco users definition.  (3) Clarified when identifying a tobacco
 user, CRS  looks back anytime prior to Report Period for health factors,
 diagnoses, and CPT codes and looks at the last documented for each.

* Alcohol Screening (FAS Prevention): (1) Added CPT codes 99408, 99409,
 G0396, G0397, and H0049 to alcohol screening definition. (2) Added
 V Measurements in PCC or BH of AUDT, AUDC, and CRFT to alcohol
 screening definition.

* Depression Screening:  Added V Measurements in PCC or BH of PHQ2
 and PHQ9 to Depression Screening definition.

* Comprehensive CVD-Related Assessment: (1) Added V Measurements in PCC
 or BH of PHQ2 and PHQ9 to depression screening definition. (2) Expanded
 CPT II range from 3077F, 3080F to 3074F-3080F for BP documented
 definition and added logic requiring CPT codes to be documented on
 non-ER visit.  (3) Added CPT codes 99406, 99407, G0375 (old code), and
 G0376 (old code) to tobacco screening definition.

* HIV Screening: (1) Renamed topic from Prenatal HIV Testing.
  (2) Added ICD-9 079.53 to HIV diagnosis definition.  (3) Clarified that
 logic for refusals of HIV screening includes a refusal of any lab
 test in site-populated taxonomy BGP HIV TEST TAX.

Logic changes to GPRA Measure Forecast Patient List include:

* Diabetes: Blood Pressure Control, Tobacco Cessation, Alcohol Screening 
 (FAS Prevention), and Comprehensive CVD-Related Assessment:  Updated
 section of list that describes what is counted for GPRA to include
 the new codes added to these topics.

Other key logic changes to non-GPRA measures include:  

* Diabetes Comprehensive Care: (1) For BP documented definition,
 expanded CPT II range from 3077F, 3080F to 3074F-3080F and added
 logic requiring CPT codes to be documented on non-ER visit.  (2) For
 Controlled BP definition, added new logic for CPT II codes, which
 are required to be documented on non-ER visits.   (3) Added CPT codes
 3044F and 3045F to documented A1c definition.

* Adolescent Immunizations: (1) Added CPT 90650 to HPV definition.
 (2) Added text clarifying logic only requires 1 contraindication for
 immunizations that require multiple doses (vs. requiring 1 contraindication
 for each dose).

* Tobacco Use and Exposure Assessment:  (1) Removed ADA code 1320
 from tobacco users and smokers definitions.  (2) Added CPTs 99406 and
 99407 to tobacco screening, tobacco users, and smokers definitions.
  (3) Added old CPT codes G0375 and G0376 to tobacco screening, tobacco
 users, and smokers definitions.

* Alcohol Screening and Brief Intervention (ASBI) in the ER:   (1) Added
 CPT codes 99408, 99408, G0396, and G0397 to ER screening, positive screen
 and BNI definitions. (2) Added new V measurements of AUDT, AUDC, and
 CRFT to ER screening and positive screening definitions.

* Cardiovascular Disease and Blood Pressure Control:  For BP documented
 definition, expanded CPT II range from 3077F, 3080F to 3074F-3080F
 and added logic requiring CPT codes to be documented on non-ER visit.

* Controlling High Blood Pressure: For BP documented definition,
 expanded CPT II range from 3077F, 3080F to 3074F-3080F and added logic
 requiring CPT codes to be documented on non-ER visit.

* HIV Screening: (1) Renamed topic from Prenatal HIV Testing.
 (2) Removed counseling/patient education numerator. (3) Added
 denominator for User Population ages 13-64. (4) Added numerator for
 counts of HIV screening for UP patients (no age range).
  (5) Added ICD-9 079.53 to HIV diagnosis definition. (3) Clarified that
 logic for refusals of HIV screening includes a refusal of any lab
 test in site-populated taxonomy BGP HIV TEST TAX. 

* HIV Quality of Care:  Added ICD-9 079.53 to HIV diagnosis
 definition.

* Sexually Transmitted Infection (STI) Screening:  Added 
 ICD-9 079.53 to HIV/AIDS definition. 

* Asthma Quality of Care:  Updated all four medication taxonomies
 with HEDIS 2008 taxonomies.

* Asthma and Inhaled Steroid Use:  Added inhaled steroid combinations
 to medication taxonomy.

* Prediabetes/Metabolic Syndrome: (1) Added V Measurement codes in
 PCC or BH of PHQ2 and PHQ9 to depression screening definition.
 (2) For BP documented definition, expanded CPT II range from 3077F,
 3080F to 3074F-3080F and added logic requiring CPT codes to be
 documented on non-ER visit (3) Added CPTs 99406, 99407, and old
 codes G0375 and G0376 to tobacco screening definition.

3. REQUIREMENTS

   a) VA Kernel V 8.0 or later

   b) VA FileMan V 21.0 or later

   c) IHS Clinical Reporting System Version 8.0 through patch 1

4. INSTALLATION INSTRUCTIONS


   In all UCIs running the IHS Clinical Reporting System (CRS):

 
    1) Save off all BGP* routines as a backup.

    2) Load the Distribution into KIDS using option 1 on the KIDS
        Installation menu.  The distribution was released in a file named
        bgp_0800.02k.

    3)  Verify the load using option 2 on the KIDS Installation menu.

    4)  Consider using options 3 and 4 on the KIDS Installation menu to
         print and compare the Transport global.

    5)  Install the distribution using option 6 on the Installation menu.

    6)  For results, use the KIDS "Install File Print" option to 
        view/print the "BGP*8.0*2" entry.

   Instructions for installing the Visual CRS (GUI):

   On any PC where Visual CRS has been installed do the following:

      1. Unzip bgp_0800.02client.zip into a directory of your choice on
         the client's PC.
         You will have 2 files:  bgp80.msi and bgp_0800.02setup.exe

      2. Double Click bgp_0800.02setup.exe that was distributed with Visual CRS.

      3. Follow the prompts on the Visual CRS Setup Screens, taking the defaults.
