The IHS Scholarship Program is now accepting applications for health profession students seeking scholarship support for the 2019–2020 academic year. Click the Apply Now button to access the online application. The instructions below will assist you in preparing the required documentation you must submit in support of your online application. We also recommend that you use the resources listed under Program Materials to guide you through the application process.

It is also recommend that you use the resources listed under Program Materials to guide you through the application process.
The Course Curriculum Verification form is included with your online application. You must provide a detailed list of your projected coursework for each academic term in which you will be enrolled during the 2019–2020 academic year. Start with Fall 2019 and end with Summer 2020.
Initial Screening for Scoring
Required documents to be eligible for scoring: Completed Online Application, Faculty Evaluations, Official Transcript(s).
You must submit your completed online application and mail your official transcripts to the IHS Scholarship Program postmarked no later than March 28, 2019 for further processing.
Initial Screening will be performed to verify eligibility for consideration of an award. Your completed online application and official transcript(s) will be needed to determine a rating score. Your application will be rated on narrative, faculty evaluations, and official transcripts.
You must submit official transcript(s) as indicated:
Undergraduate level: Must submit official transcript(s) that indicate a minimum of 24 credit hours of college coursework to be completed by June 1, 2019. You are required to send official transcripts from every college/university attended within the last 7 years.
Graduate level: Must submit official transcript(s) from last degree obtained and current coursework/transcript, if presently enrolled.
Selected applicants will be notified to submit the following additional documents to complete the selection process within 30 days upon notification.
- Letter of Acceptance
- BIA Form 4432 or State Recognized Tribal Card/Documents
- Curriculum for Major
- Declaration of Federal Employment-OMB Form 3206-0162
- Addendum OF 306 Form-OMB Form 0917-0028
Applicants not selected for an award will be notified by mail. Award Letters will be sent to potential scholarship recipients by the end of July.
