Community Health Representative (CHR)
The CHR Data Mart provides access to core CHR reports for all sites exporting data to the National Data Warehouse and includes the ability for senior CHR leadership and program staff to access the most critical performance data for the program including:

Division of Environmental Health Services (DEHS)
The Notifiable Disease and External Cause of Injury (NDECI) Data Mart and Reporting System tracks data for these five specific categories:
  • Asthma - any ICD9 code with the first 3 digits of "493"
  • Injury - ICD9-CM Codes E800-E999 with the exception of Suicide related injuries
  • Intestinal Disease - ICD9 CM codes 001-009
  • Notifiable Disease - any ICD9-CM codes in the following code ranges: 001-139, 460-519, 680-709, V01-V02 and V07-V09
  • Vectorborne Disease - ICD9 CM codes 020-027, 060-066 and 080-088
Division of Oral Health (DOH)
The Dental Data Mart utilizes data from the Resource Patient Management System (RPMS) Patient Care Component (PCC) exports, Contract Health Services (CHS) data received from the Fiscal Intermediary, data export from the CHS/MIS application and non-RPMS data provided through the "simplified" export file to provide the following reports:

Eye Care Data Mart
The Eye Care data mart makes full use of data mapped and exported to the IHS’ National Data Warehouse project. For this "version 1" application, nine (9) major eye care categories have been defined. These categories include:

  • Diabetic Retinopathy
  • Eye Glasses Dispensing
  • Glaucoma
  • Macular Degeneration

National Immunization Reporting System (NIRS)
The NIRS web-based application is designed to collect quarterly immunization data from each participating IHS facility. Data entered through this system are used to develop Area and National level immunization reports regarding the following vaccination groups:
  • 3 to 27 Month Old Vaccinations
  • Adolescent Vaccinations
  • Adult Report
  • Healthcare Personnel Influenza Vaccinations
  • Influenza Vaccinations
  • Pediatric Influenza - Annual 3rd Quarter Immunization Report
  • Two Year Old Vaccinations
Baby-Friendly Hospital Initiative (BFHI)
The “Baby Friendly Hospital Initiative” (BFHI) is a global program launched in 1991 by the United National Children’s Fund and the World Health Organization to implement practices that protect, promote and support infant breastfeeding. This initiative is a quality improvement activity that will increase breastfeeding rates through improved maternal support for starting and continuing breastfeeding.

Pharmacy Workload Data Reporting System (PWDRS)
The purpose of the version 1 Pharmacy Workload Data Reporting System (PWDRS) application is to provide the Office of Clinical and Preventive Services (OCPS) Pharmacy program the ability to generate national or regionally aggregated data for all Pharmacy related services and basic drug prescription information.
