New Agency Initiatives

Partnership for Patients (P4P) Initiative

The Partnership for Patients initiative is an agency-wide initiative to reduce hospital readmissions by 20% by the year 2013. The P4P initiative has identified the following focus areas: 1) Adverse Drug Events (ADE); 2) Catheter-Associated Urinary Tract Infections (CAUTI); 3) Central Line Associated Blood Stream Infections (CLABSI); 4) Injuries from Falls and Immobility; 5) Obstetrical Adverse Events; 6) Pressure Ulcers; 7) Surgical Site Infections; 8)Venous Thromboembolism (VTE); 9) Ventilator-Associated Pneumonia (VAP); 10) Other Hospital Acquired Conditions.

The agency is still developing a strategy and identifying potential data sources that could be used to assist with this effort. The NPIRS program has done some preliminary work with the National Public Health Nursing (PHN) program to see how utilizing PHN "home visits" could help reduce hospital readmissions. Please watch this site closely as the agency strategy is developed and supporting data is made available.
