Background:
Standard plywood subflooring placed on wood joist at 400 and 600 mm spans; no other special details or specifications were required by the contract. Essentially this is a residential standard subfloor and exhibits deflection and a squeaking noise when walked on.

Lessons Learned:
A commercial floor must be specified to handle the traffic in a Health Center, especially in the public areas.

Recommended Action:
If a plywood floor is used in a Health Center, it should be detailed with two layers (structural and leveling subfloor), non-overlapping joints or joint support between joists and fastened with glue and screws.
