Background:
Only one chiller was ever online, the #2 chiller never came online. The chillers were piped in parallel, and the water from the two chillers mixed before re-entering the building. These chillers were designed to run together, but were being controlled as a primary unit and a redundant unit. So as chiller #1 cooled the water, chiller #2 heated it back up.

Lessons Learned:
Sometimes the subcontractors for individual systems do not ensure that their system is working correctly with other systems to accomplish the overall design objective.

Recommended Action:
Verify that subcontractors are not only strictly following their own specs, make sure the systems they are working on are compatible with all other systems in the facility.
