Background:
Due to certain design requirements there is no large door to the mechanical/boiler room, which has been located in the main hospital building. The exterior wall of the boiler room also contains extensive piping and equipment. The only way to remove major equipment is to first remove a large precast concrete access panel in the boiler room exterior wall.

Lessons Learned:
Avoid this situation by placing the mechanical/boiler room in a separate or semi-detached building. Proper access doors can be strategically located for convenient equipment removal.

Recommended Action:
If access and removal of equipment becomes a problem in design, require the A/E to develop a cartage plan which indicates access and removal path for all equipment.
