Summary:

Given that tobacco use increases risk of periodontal disease, oral cancer, and other adverse oral and general health effects, it is important that dentists, dental hygienists, and dental staff acquire effective knowledge and skills for treating tobacco use and dependence in the dental setting. This lecture will address the epidemiology of smokeless tobacco [ST] use among high school males in rural areas of California, and the evidence for brief interventions in the dental setting. The lecture will focus on the challenges to cessation, brief tobacco cessation interventions, as well as more extensive behavioral and pharmacologic treatments, to assist patients with stopping their tobacco use. Motivational interviewing will be presented as an easy, effective, non-threatening method for dealing with patient resistance and increasing patient motivation and compliance with recommended evidence-based approaches to tobacco cessation. This program is designed to include the entire dental team. In order for a practice to be effective in treating tobacco use and dependence, every member plays an important role in the routine identification, documentation and treatment of every tobacco user seen for dental care.

Learning Objectives:

By the completion of this lecture each participant will be able to:

1. Describe the prevalence of smokeless tobacco use among high school males in California.
2. Explain the physical, psychological, behavioral, and sensory aspects of nicotine addiction.
3. Apply the National Cancer Institute’s 5 A’s [ask, advise, assess, assist, arrange] when treating patients for tobacco use and dependence.
4. Assist patients with tobacco use cessation based on their readiness to quit.
5. Describe the “spirit” and key skills of Motivational Interviewing.
6. List the four “opening strategies” of Motivational Interviewing represented by the acronym, OARS to elicit change talk
7. Explain the “Elicit-Provide-Elicit Model” brief intervention for assisting a patient who is ready to quit.
8. Explain at least 2 skills for coping with temptation to use ST.
9. Explain the proper use of the following aids for cessation [including dosing, potential side effects, and precautions]:
a. Nicotine polacrilex gum
b. Nicotine polacrilex lozenge
c. Nicotine transdermal patch
d. Nicotine nasal spray
e. Nicotine inhaler
f. Bupropion SR
g. Chantix

Calvin Hedrick, Health Education Specialist –Tobacco Education and Prevention Technical Services [TEPTS] at CRIHB and Lalani Ratnayake, CA Dental Support Center Coordinator will present the afternoon session.

1. Apply Motivational Interviewing skills in counseling dental patients in tobacco cessation.
2. Explain the use of the stages of change for use in tobacco cessation counseling.
3. Provide resources and support to quit smokeless tobacco use and prevent relapse among patients.
4. Participants will have the knowledge of tobacco cessation protocols for the dental practice

Tuition Policy:

Tuition must be paid in full 8 weeks prior to the start date of any course. Request for refunds must be received in writing at least two weeks before the course begins. For each refund request, there will be an administrative charge of $100. No refunds will be made to registrants who fail to attend a course. If IHS CDE program cancels a course, then 100% of the tuition will be refunded.
