BACKGROUND:

This transmittal forwards Part 3, Chapter 9, Ancillary Clinical Services, of the Division of Indian Health Manual. This chapter sets forth the Division of Indian Health policy and procedures regarding Therapeutic Sterilization, Therapeutic Abortion, and Family Planning.

The two forms to be used in connection with these procedures are:

HSM-83, (formerly PHS-3808) "Consent for Therapeutic Sterilization, Procedures," and

HSM-84, "Consent for Therapeutic Abortion."

An initial supply of these forms will be forwarded from the usual production sources. Future requests should be submitted in the regular manner.
