Special Diabetes Programs for Indians - Diabetes Prevention Program and Healthy Heart Initiatives Information

FY 2015 CONTINUATION APPLICATION (Budget Period 9/30/15 - 9/29/16)

The application package and instructions for FY 2015 are now available. Carefully read the Continuation Application Instructions as you begin to prepare the application.

Steps for Submitting the Continuation Application on GrantSolutions

  1. Log on to GrantSolutions.gov. Exit Disclaimer: You Are Leaving www.ihs.gov
  2. Click "Apply for Non Competing Award", this link will be available under "My Grants List" when you press the '+' (plus sign) near the left side of the screen.
  3. Enter all the application information in all of the different forms. Questions about the online forms may be directed to the Division of Grants Management (DGM) at GrantsPolicy@IHS.gov.
  4. After completing all of the forms and uploading any attachment files that are needed to finish the application, click "Verify Submission".
  5. Click "Final Submission". Press "OK" when you receive the prompt "Are you sure you want to submit application?".

Application Deadline

The due date to submit the FY 2015 Continuation Application is June 1, 2015.
