Code Type of Facility Definition
01   Hospital  A permanent facility which contains inpatient beds, organized staff including physician services, continuous nursing services and that provides comprehensive health care including diagnosis and treatment. 
02   Health Center  A facility physically separated from a hospital, with a full range of ambulatory services including physician services, nursing, pharmacy, laboratory, and x-ray.  Services are available at least 40 hours per week for ambulatory care. 
03   School Health Center  A facility which is a health center and primarily serves students. 
04   Health Station  A facility, physically separated from a hospital or health center, where primary care provider services are available on a regularly scheduled basis but for less than 40 hours per week. 
05   Administration  A facility where health care management activities are performed and where treatment given on site is only ambulatory with visiting providers less than one day per week that provides limited primary care or other health services. 
06   Dental Clinic  A healthcare facility where direct and/or contract dental services are provided. 
07   Doctor Office  No longer used 
08   Extended Care (Elder Care)  A daytime facility where those who are retired may receive care and support, returning to their home environment at night. 
09   Other  Any facility which does not clearly fit the definitions of the other facility types in this table. 
10   Nursing Home  No longer used; see Long Term Care 
11   Alaska Village Clinic  A healthcare facility located exclusively within Alaska Native communities and typically leased by IHS; care is usually provided by a community health aide or practitioner. 
12   Health Location  An ambulatory care location operating with visiting providers less than one day per week in a remote community that provides limited primary care and dental services. 
13   Alcohol Substance Abuse Treatment Facilities  A healthcare facility where alcohol or substance abuse care is provided. 
14   Behavioral Health Facilities  A healthcare facility where behavioral health care is provided. 
15   Assisted Living Center  A facility where contract health care is authorized from or where community health services staff are stationed such as Public Health Nurses (PHN), etc. No direct primary care services are provided by this facility nor are APC's generated. These sites may use non-APC or non-PCC reporting systems; i.e., CHS, PHN, etc. 
16   Standalone Urgent Care Facility  A facility physically separated from a hospital where urgent health care is provided. Weekend and evening hours are expected. 
17   Contract Inpatient  A permanent facility which contains inpatient beds, organized staff including physician services, continuous nursing services and that provides comprehensive health care including diagnosis and treatment. IHS patients are seen on a contract basis in this facility.A direct feed of electronic medical data goes from this facility to the National Data Warehouse. 
18   Psychiatric Inpatient  A permanent facility which contains inpatient beds, organized staff including physician services, continuous nursing services and that provides exclusively psychiatric or behavioral health care including diagnosis and treatment. 
19   Standalone Ambulatory Surgery  A facility physically separated from a hospital where ambulatory surgery and noninvasive imaging procedures such as colonoscopy are provided. 
20   Long Term Care  A healthcare facility where long term health care is provided with 24 hour nursing care on site. 
21   OPERATING ROOM  AN OPERATING ROOM FOR DENTAL PROGRAM WHERE DENTAL SERVICES ARE PROVIDED 
22   PEDIATRIC CLINIC  A PEDIATRIC CLINIC FOR DENTAL PROGRAM WHERE DENTAL SERVICES ARE PROVIDED 
23   OUTREACH PROGRAM  OUTREACH PROGRAM CODE FOR DENTAL PROGRAM WHERE DENTAL SERVICES ARE PROVIDED 
24   Youth Regional Treatment Center  A residential treatment facility that provides substance abuse services and other behavioral health interventions for Native American and Alaskan youth.
