2013 CHR Basic and Refresher Training Announcement
Welcome! Please review all information below before applying. You must be a CHR employed by a Tribe, Urban health program or IHS facility to apply.
CHR Basic is for CHRs who have not received a certificate of completion for that course. CHR Refresher is for those CHRs who haven't had a Refresher Training in the past 36-48 months.
Fees: No tuition fees are charged to Tribes that have left their IHS Headquarters (HQ) CHR shares and IHS pays for travel, per diem and associated costs for a CHR approved to attend the training.
Tribes that have taken their IHS HQ CHR shares and Urban health programs are responsible for tuition, travel, per diem and associated costs for a CHR approved to attend the training. The tuition fee amounts are:
Registration Requirements: Applicants must complete the online application form and include proof of current CPR/AED (adult, child, infant) certification at the time of application to be considered. Your application is not complete until a faxed copy of your CPR/AED certification has been received at (405)-951-3902. To confirm your fax has been received, call (405) 951-6009. It's YOUR RESPONSIBILITY to confirm that the fax was received. Please note: CPR/AED class will not be provided onsite.
We encourage all CHRs who need the training to apply. For those of you who regrettably aren't selected, your numbers will help us demonstrate unmet training needs.
To increase your opportunity to be accepted in the class, please apply early with your proof of current CPR certification.
Registration Opens: Thursday, October 18, 2012 at 11 AM EST/10 AM CST/9 AM MST/8 AM PST.
Registration Closes: Friday, November 2, 2012
Acceptance Notification Date: Applicants who have been accepted as students will be notified by the Oklahoma City Area Inter Tribal Health Board (OCAITHB), the contractor, no later than Friday, November 9, 2012.
Required Assignment: Students approved for CHR Basic Training will receive instructions for pre-course preparation prior to the onsite training. Applicants must complete the assigned workbook modules and return the assignments to the OCAITHB to be approved to attend the onsite training in Norman, OK.
Lodging: Students who are approved to attend do not need to make their own hotel reservations. OCAITHB will create a rooming list for all students and provide it to the hotel.
Please note: Only students who have been notified of acceptance are allowed to attend the onsite training.
Location: Norman, OK
All incidentals (i.e., room service, phone, restaurant, movies, dry cleaning services, etc.) will require a cash or credit card deposit. Students will be responsible for all incidental charges made to the room and must pay those charges upon checking out of the hotel. OCAITHB and IHS are not responsible for any unauthorized charges or destruction of hotel property.
The sleeping room rate for the training is $77 per night plus tax.
