BACKGROUND:
This transmittal forwards amendment number 1 to Part 3, Chapter 14 of the Indian Health Manual. The amendment provides for a change in Form PHS-2603, Clinical Records - Report of Medical Examination of School Children.
CHANGE:
Remove and destroy pages 3-14.4 and 3-14.5 and substitute the enclosed pages 3-14.4, 3-14.5 and 3-14.5c.
