The Foundation Series is a quarterly series of web-based seminars where we share tools an ideas for improving care from across the Indian health system that have been tested by teams in the Improving Patient Care Collaboratives. The series provides organizations with hands-on learning and prepares participants with tools and approaches they can use in their out-patient clinical settings. The series is organized around three important topic areas:
Understanding the experience of care for our patients and communities;
Building relationships for care that are evidenced-based;
Leadership for high-quality health care.
The Foundations Series is designed to provide education and training to non-IPC sites in the Indian health system in order to lay the groundwork for participation in IPC Collaborative 5. These webinars can also provide orientation to new team members and leadership at existing IPC sites.
This opportunity is intended for:
Frontline staff, senior leaders, community members, and Tribal leaders from sites that are not currently involved in IPC who are interested in learning more about the tools and change concepts shared through the Improving Patient Care Collaborative.
New staff from current IPC/Quality and Innovation Learning Network sites that need orientation.
Current IPC sites with frontline staff and leadership ready to test and use what they learn.
Areas of focus
The calls will focus on key areas of the change package including: leadership; care teams and improvement teams; empanelment; access to care; and data including the Model for Improvement and critical measures.
Activities will include
The calls are held virtually using Adobe Connect. The calls are interactive and feature presenters from the IPC collaborative sites as well as topic experts and faculty. Assignments are given with opportunities for feedback and mentoring from other teams as well as faculty.
