Search, Sort and Download tips  click on icon to open help Search Code (autotextsuggest field), Provider (empty field), Status (drop down list), Wl Flg (drop down list), Prm Prov Flg (drop down list) and Definition (empty field) columns only or any combination of these columns.

Autotextsuggest pulls values from the field in the database that begin with the string (not case sensitive) entered by the user and displays them as drop down list options. These values populate as the user types. Drop down list searches for results that are an exact match with the value selected by the user. From drop down list, select value 'Empty' to select results where the field is not populated. Empty field searches for results that begin with, include or end with the string (not case sensitive) entered by the user.

To Sort the columns in a table click the column header(s) that is bolded and underlined. Columns will sort ascending (any unpopulated will display at the top) or descending.

E4  PHYSICAL THERAPY STUDENT  Active  An individual, who is enrolled in an accredited program in Physical Therapy in the United States or Canada, is under the supervision of a licensed Physical Therapist and is participating in the accredited program’s academic clinical educational curriculum. 
E3  EMERGENCY ROOM TECHNICIAN  Active  An individual who assists emergency department physicians and midlevel providers. Duties include but are not limited to: taking vital signs, performing phlebotomy, specimen collection, performing EKGs, dressing changes, casting and splinting, suturing and IV insertion and patient transports. 
E2  ECHOCARDIOGRAPHER  Active  An individual who specializes in echocardiography or vascular technology run noninvasive tests using ultrasound instrumentation, such as Doppler ultrasound. 
E1  CARDIOVASCULAR TECHNICIAN  Active  An individual who performs electrocardiograms (EKGs), stress testing, and Holter monitors. 
D9  CARDIOVASCULAR TECHNOLOGIST  Active  An individual who assists physicians in diagnosing and treating cardiac (heart) and peripheral vascular (blood vessel) ailments. Cardiovascular technologists may specialize in three areas of practice: invasive cardiology, echocardiography, and vascular technology. 
D8  OCULARIST  Active  An individual skilled in the design, fabrication, and fitting of artificial eyes and in the making of prostheses associated with the appearance or function of the eyes. 
D7  TOBACCO TREATMENT SPECIALIST  Active  An individual who trained in tobacco treatment who conducts tobacco treatment counseling, cessation programs and services. 
D6  APRN PSYCHIATRIC NURSING SPECIALTY  Active  An individual who is licensed/credentialed by a state as an independent practitioner (nurse practitioner) may diagnose disease and provide appropriate treatment including prescribing medications up to and including Schedule II Narcotics. Must have a DEA certification and number to write prescriptions for controlled substances. 
D5  LICENSED CLINICAL SOCIAL WORKER  Active  A person who has a Masters Degree in social work from an accredited college or university program and who has met the requisite state requirement of supervised clinical practice hours and is licensed as an independent practitioner qualified to provide counseling and psychotherapy to individuals, families or groups, arranging or supervising the provision of social services and interpreting the psychosocial aspects in the situations of individuals, families and groups. 
D4  LICENSED PROFESSIONAL COUNSELOR  Active  A person who has a Masters Degree in mental health counseling, psychology or marriage and family therapy who is licensed by a state as a Licensed Professional Counselor qualified to provide assessment and diagnosis of behavioral health disorders, counseling, crisis intervention, case management, and/or marriage and family counseling. 
D3  EARLY CHILDHOOD INTERVENTION SPECIALIST  Active  An individual who works with at risk children, infants and toddlers who present with a mild, moderate to severe developmental delay, thus impacting their educational development.  Intervention is based on standardized measures.  This individual provides strategies to caregivers, continuous monitoring of the child, infant or toddler's development.  The Early Childhood Intervention Specialist may refer patient out for developmental assessment,  Speech-Language Therapy, Occupational Therapy, Physical Therapy, Behavioral Therapy or psycho educational testing through a Child Psychologist. 
D2  BEHAVIOR ANALYST  Active  An individual who has a Master's degree as a Board Certified Behavior Analyst and provides individualized service to assess and remediate identified behavioral challenges, which inhibit successful integration into the community. This individual may not provide cognitive therapies or psychological testing, neuropsychology, psychotherapy, sex therapy, psychoanalysis, hypnotherapy, and long-term counseling as treatment modalities. 
D1  BEHAVIORAL HEALTH NURSE PRACTITIONER  Active  An individual qualified by an approved postgraduate program in nursing and licensed by the practicing state, commonwealth or territory to practice professional behavioral health nursing including prescribing in the specialty. 
C9  BEHAVIORAL HEALTH AIDE/PRACTITIONER  Active  An individual having completed the requirements of the Community Health Aide Program at the Behavioral Health Aide I, II, III, or Behavioral Health Practitioner level as outlined by the Community Health Aide Program Certification Board. This individual provides prevention, early intervention, and aftercare mental health and substance abuse services. BHA/Ps work under indirect or general supervision of a licensed behavioral health provider or licensed mental health clinician as outlined in the federal Community Health Aide Program Certification Board Standards and Procedures. C8  COMMUNITY HEALTH AIDE/PRACTITIONER  Active  An individual having completed the requirements of the Community Health Aide Program at the Community Health Aide I, II, III, IV or Community Health Practitioner level as outlined by the Community Health Aide Program Certification Board. This individual provides acute care, emergency care, follow-up care, and preventative services. CHA/Ps work under indirect or general supervision of a licensed provider outlined in the federal Community Health Aide Program Certification Board Standards and Procedures. (ALASKA) 
C7  FITNESS SPECIALIST  Active  An individual who is a fitness specialist or fitness instructor who conducts exercise/fitness classes for patients at risk for various medical problems. 
C6  MASSAGE THERAPIST  Active  An individual who is a licensed massage therapist who provides head, neck, hand and full body massages for various medical problems. 
C5  BEHAVIORAL HEALTH STUDENT  Active  An individual who is enrolled in or a graduate from a United States accredited program for a 1) Doctoral degree in Psychology; 2) Master’s or Doctoral degree in Social Work; 3) Advanced Practice Registered Nurse (APRN) master’s degree with psychiatric nursing specialty or 4) COSTEP/summer extern. 
C4  RESPIRATORY THERAPY TECHNICIAN  Active  An individual with formal training in the respiratory therapy field who works under the direct supervision of a licensed Respiratory Therapist. 
C3  MEDICAL ASSISTANT  Active  An individual with specialized general medical office training and certification from an accredited Medical Assistant training program. The medical office generalist can perform duties that include initial registration and triage, administration and charting, and performance of direct patient care services of a routine nature as established by clinic protocol. The medical assistant is supervised by a physician or licensed healthcare practitioner and works in collaboration with nursing and other clinical personnel. 
C2  PHYSICAL THERAPY TECHNICIAN  Active  An individual with formal training in the physical therapy field who works under the direct supervision of a licensed physical therapist. 
C1  PHARMACY STUDENT  Active  An individual who is enrolled in or a graduate from, a United States or Canadian accredited college of pharmacy, is under the supervision of a registered pharmacist and meets one or more of the following criteria: 1) licensed/registered as an intern by a state board of pharmacy, 2) participating in a United States or Canadian accredited college of pharmacy’s academic experiential program or 3) COSTEP/summer extern. 
B9  CHAPLAIN  Active  An individual qualified to receive referrals to serve patients within their scope of service and document the services rendered. 
B8  SURGICAL TECHNICIAN  Active  An individual with or without certification who performs technical work in either a surgical environment or in the operating room, under the supervision of a surgeon or registered nurse or nurse manager. 
B7  DENTAL HEALTH AIDE THERAPIST  Active  An individual with a Diploma in Dental Therapy (DipDentTher) or the Postgraduate Diploma in Dental Therapy (PGDipDentTher), or as an endorsement for the Bachelor of Health Sciences (BHealSc) degree from an institution approved by the Community Health Aide Certification Board and recertified every two years and must obtain continuing education and clinical evaluation during the interim.  This individual provides diagnosis, preventive, restorative and surgical services working under indirect or general supervision of a licensed dentist within a personally tailored scope of practice.  (ALASKA)
