Overview

The Special Diabetes Program for Indians (SDPI) provides funding for diabetes treatment and prevention activities and services in American Indian and Alaska Native communities. SDPI Community-Directed Programs implement diabetes-related activities and services based on local needs and priorities.

Applying for SDPI FY 2016 Community-Directed Funding

Application materials and resources will be available on this website, including forms/templates, training opportunities, and tools for planning, implementation, and evaluation. Before applying for funding, be sure to:

  1. Read the entire Funding Opportunity Announcement (FOA) when it is available, with particular attention to the Eligibility Information, Grantee Cooperative Agreement Award Activities, and Application and Submission Information.
  2. Review the information on each of the tabs above, with particular attention to the Best Practices and their related requirements.
  3. The deadline date will be for ALL applicants, regardless of previous SDPI grant budget cycle. Incomplete or late applications will not be accepted.
