Summary: Chapa-De is a non-profit community outpatient health center located in Norther CA. Our mission is to advance the health and well-being of American Indians and low income individuals living in our communities by providing convenient access to high quality, compassionate care. We provide comprehensive patient-centered primary care with a broad spectrum of services including adult and pediatric primary care, nutrition and health education, womens health/pre-natal care services, dental care, orthodontia, mental health and substance abuse counseling, psychiatry, optometry, and pharmacy.

Chapa-De Indian Health is seeking a part-time Board Certified or Eligible Family Practice Physician to be part of our team that will provide comprehensive patient care at our clinic located in Auburn, CA. Tentative Schedule will be Monday through Wednesday from 5 PM to 8 PM and Thursday from 1 PM to 8 PM. Scope of work includes adult pediatric primary care, chronic illness and disease management minor surgeries and procedures, and all other areas of family medicine.
Duties: 1. Provide direct medical services to Program patients. Interdepartmental medical consultation as requested. 3. Clinical supervision of mid-level staff, resident physicians, FNP/PA and medical students as needed. 4. Participate in the development of medical services, including protocols, standard procedures, and management of patient care related activities. 5. Serve as liaison with the local medical community. Initiate appropriate specialty referrals. 6. Provide leave coverage, if necessary, between the Auburn and Grass Valley sites as agreed upon with the Medical Administrator. 7. Advise Medical Administrator of any problems that can affect the efficiency of the Medical Department. 8. Provide required Federal and State Ambulatory Patient Care Data on appropriate forms and follow Indian Health Service health care maintenance guidelines. 9. Record patient chart entries in Subjective, Objective, Assessment, and Plan [SOAP] format. 10. Complete appropriate documentation necessary for Data and Billing Advise nursing staff of appropriate response to patient phone calls. 12. Other duties and responsibilities as assigned
Qualifications: 1. Licensed California Physician Board Certified in Family Practice preferred [Pediatric/ Internal Medicine specialty will be considered, if applicant possesses a broad Experiential background]. 2. Two years clinical experience in the direct delivery of primary care. 3. Experience with EHR or equivalent. 4.BLS certification required. 5. Sensitive to American Indian culture and health needs. Prior experience in cross-cultural medicine desirable.
