Summary: Chugachmiut is the Tribal consortium created to promote self-determination to the seven Native communities of the Chugach Region. We provide health and social services, education and training, and technical assistance to the Chugach Native people in a way which is acceptable to Native cultural values and tradition in order to enhance the well-being of our people by continuing to strengthen the tribes and increase self-determination opportunities for community operated tribal programs.

Provides professional nursing services to accomplish the individual patient plan of care; independently and consistently using the nursing process to safely, therapeutically, and efficiently care for a full assignment of patients with subtle and sometimes unidentified needs. Support the operations of the Chugachmiut Health Services programs.The functions of the RN Case Manager are to independently and consistently use nursing knowledge, collect patient health data, analyze the data to determine diagnoses, identify expected outcomes individualized to the patient, develop a plan of care that prescribes interventions to attain the expected outcomes, implement the interventions identified in the plan of care, and evaluate the patient's progress toward attainment of the outcomes, working collaboratively with the healthcare team. Focus should be placed on preventative care services as well as necessary follow up on various individuals with acute and chronic conditions across the region.
Duties: 1. Independently and consistently uses nursing knowledge, collect patient health data, and analyze data to determine diagnoses, while identifying expected outcomes which are individualized to each patient. 2. Work closely with our tribal healthcare partners across the state to coordinate care across the continuum for our patients. 3. Collaborate with the patient, family and other health care providers to formulate the plan of care and goals as well as assist the patient and family to identify and obtain the appropriate and available services for their health care needs. 4. Utilizes the ANA Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as the foundation for nursing practice. 5. Interacts with and contributes to the professional development of peers and others practicing in a Shared Governance model of nursing practice. 6. Makes decisions and takes actions on behalf of patients, be determined in an ethical manner, and deliver care in a nonjudgmental and nondiscriminatory manner that is sensitive to patient diversity preserving patient autonomy, dignity, and rights. 7. Performs nursing assessments, interventions, and evaluations. 8. Considers factors related to safety, effectiveness, evidence based-practice, and cost in planning & delivering patient care, and assists the patient and family in identifying and securing appropriate and available services to address health-related needs. 9. Delivers timely and effective nursing care according to established standards including physical, social and emotional care. 10. Collaborates with the patient, family, and other health care providers in the formation of overall goals and the plan of care, as well as in decisions related to care and the delivery of services. 11. Educate patients in a wide variety of health care subjects such as disease prevention, life style issues, basic health care maintenance, and self-administration of medications. 12. Perform patient follow-up including notifying patients of laboratory results, medication refills, scheduling referral appointments, maintaining charts and records, and responding to patient telephone calls. 13. Utilize the ANA Scope and Standards of Practice and code of Ethics for Nurses. 14. Assist physicians and other healthcare providers with procedures. 15. Directs the day to day activities of Medical Assistants and provide essential training as required for Medical Assistants to perform daily job duties. 16. Effectively communicates with patients and families in a culturally sensitive manner. 17. Develops and assists with Performance Improvement initiatives. 18. Serves as a clinical expert and resource on case management to Chugachmiut Health Services. 19. Initiates and develops in-service training to keep staff updated on issues pertaining to health wellness, disease management and coordination of care. 20. Attends and participates in Clinical Service team meetings, staff meetings, and other committee efforts as appropriate. 21. Prepares patient education materials as needed. 22. Operates, monitors, and maintains patient care equipment. 23. As requested participates in and/or provides health education opportunities for beneficiaries and communities in conjunction with other Chugachmiut staff. Activities may include providing health education and screening services at schools, health events, and other community events. 24. Ability to provide services via telehealth. 25. Ability to travel to remote village clinics to provide direct care. 26. Maintains cooperative working relationships and communication with other health care providers and health care agencies as appropriate for service provision and coordination. 27. Ensures the maintenance of timely and accurate patient records to reflect each patient's health status and risk factors and participates in training for Electronic Health Record; 28. Complies with all Chugachmiut personnel and Health Services Division policies, licensed provider policies, regulations, and guidelines for patient care and maintains credentials in good standing and appropriate to level of practice. 29. Complies with statutes and regulations from the State of Alaska, Division of Occupational Licensing, for Registered Nurses. 30. Performs other duties as assigned.
Qualifications: • Alaska license in good standing as a Registered Nurse • Current BLS required • Experience in primary care services preferred • Experience in rural health care setting preferred • Experience in tribal health setting preferred
